1. Applicant's Alabama General Contractor's License is applicable for the scope and value of work noted

2. Applicant has been in business under current name for at least Five (5) years as of the scheduled bid
date. Provide a statement on the applicant's letterhead certifying the month and year that the firm

3. Applicant's average annual revenue for the past Five (5) years is at least $10M.

4. The applicant's current projects under contract should not be more than 1.75 times its average yearly

1. Applicant's firm has successfully completed one (1) or more recreational facilities within the last three
(5) years, similar to the project described at the beginning of this document. Provide description to
include project name, description of work that documents similarity to this project, location, owner

D. PROJECT PERSONNEL
1. PROPOSED PROJECT MANAGER:

The last day to submit a Request for information or clarification for this RFQ May 1st.
Prequalified bidders will be notified by May 11, 2026.
Bid documents are currently scheduled to come out late May 2026.

Total construction time from award 215 Calendar days
The City of Decatur reserves the right to revoke the applicant's prequalification approval
or consider a bid nonresponsive, if it is determined that incorrect information was
provided by a bidder in order to prequalify for this project.
