1. Applicant’s Alabama General Contractor’s License is applicable for the scope and value of work noted
above. Please attached copy of current State of Alabama General Contractor’s License. (True/False)
2. Applicant has been in business under current name for at least Five (5) years as of the scheduled bid
date. Provide a statement on the applicant’s letterhead certifying the month and year that the firm
was founded. (True/False)
3. Applicant’s average annual revenue for the past Five (5) years is at least $10M.
Provide a statement on the applicant’s letterhead certifying the annual revenue for each of the five
(5) previous years. (True/False)
4. The applicant's current projects under contract should not be more than 1.75 times its average yearly
revenue from the last five years. For example, if a company's average annual revenue over the last five years is $1
million, then the current value of all their projects under contract should not exceed $1.75 million Provide a statement
on the applicant’s letterhead certifying the amount of work currently under contract. (True/False)
5. The current liability insurance limits of the applicant must reflect financial strength and responsibility
must meet the insurance requirements of the project. See Attached Insurance Requirements. Attach a
letter from your Insurance Carrier verifying that the project insurance requirements are obtainable.

1. The applicant can provide a surety bond for the project in the estimated amount of $15 million and
bonding company has a Best Rating of A- or better and a United States Treasury limit at least equal to
the estimated contract sum of this project. Provide letter from surety company indicating applicant’s
ability to bond project. (True/False)
C. SIMILAR WORK
1. Applicant’s firm has successfully completed one (1) or more recreational facilities within the last three
(5) years, similar to the project described at the beginning of this document. Provide description to
include project name, description of work that documents similarity to this project, location, owner
with contact name and phone number plus the month and year of substantial completion. (True/False)
D. PROJECT PERSONNEL
1. PROPOSED PROJECT MANAGER:
a. Applicant’s Project Manager can be used on a part time basis on the project but at no time will the PM
