EVALUATOR TO IMPLEMENT A
COMPREHENSIVE OPIOID, STIMULANT, AND
SUBSTANCE ABUSE SITE-BASED PROGRAM (COSSAP)
RFP NO. 2021-0186-CSD/KE
(NM State Commodity Codes 91803)
Santa Fe County requests proposals from qualified firms to provide a local evaluator to
implement a comprehensive opioid, stimulant, and substance abuse site-based program
(COSSAP). This position is a grant funded position that will collaborate with the States cross-
site evaluation team, performing data collection and analytic responsibilities. All proposals
submitted shall be valid for ninety (90) days subject to action by the County. Santa Fe County
reserves the right to reject any and all proposals in part or in whole. A completed proposal can be
submitted in a sealed container, indicating the proposal title and number along with the Offeror's
name and address clearly marked on the outside of the container or delivered electronically
utilizing a DropBox. All proposals must be received by 2:00PM on Thursday, May 20, 2021,
at the Santa Fe County Purchasing Division, 102 Grant Avenue, Santa Fe, NM 87501. To
combat the spread of COVID-19, it is preferred that the submission of Proposals be delivered

II. INTRODUCTION
A. PURPOSE OF THIS REQUEST FOR PROPOSAL
The County Community Services Department received a grant from the NM Behavioral
Health Services Division to implement a Comprehensive Opioid, Stimulant, and
Substance Abuse Site-based Program (COSSAP) within Santa Fe County. The grant
requires contracting with a local evaluator to collaborate with the State of New Mexico
cross-site evaluation team and to perform data collection and analytic responsibilities.
Santa Fe County desires to procure the services of an evaluator, for the local Santa Fe
County Site, that meets all of the qualifications to provide the duties described in this RFP.
B. SCOPE OF WORK
1. Description of Scope of Work and Deliverables:
a. Participate in COSSAP implementation planning meetings and processes.
b. Access, analyze and summarize community data for use in the implementation
planning process. Data may include drug related arrest data, drug overdose death
data, overdose related emergency department data, drug/alcohol related 911 call
data, jail census data, treatment data, and/or housing capacity data.
c. Develop local evaluation plan, in consultation with COSSAP project manager,
other staff and partners, Policy Coordinating Committee, and cross-site evaluation
team.
d. Customize cross-site evaluation forms and procedures to align with local data
systems and program design, while optimizing data quality and adherence to
informed consent, privacy and confidentiality requirements.
e. Train and coach program staff in evaluation data collection and entry.
f. Conduct monthly data quality checks of COSSAP participant and program data.
g. Prepare and submit relevant client services data to cross-site evaluation
team monthly.
h. In collaboration with the COSSAP Program Manager, complete the federal online
Performance Measurement Tool (PMT) each quarter.
i. Prepare quarterly COSSAP program evaluation key indicator reports for the
LEAD Policy Coordinating Committee and other local stakeholders.
j. Obtain, analyze and report drug-related arrest data each quarter to the Policy
Coordinating Committee. As needed, work with local law enforcement agencies
