Background and Project Objective
Since October 2024, Grand County Public Health (GCPH) has convened a
diverse, cross-sector coalition of medical, dental, and behavioral health
providers to address gaps identified in the Community Health Assessment
(CHA). While the coalition has successfully established rapport and identified
systemic barriers, professional facilitation is required to transition from a
foundational assembly phase into an action-oriented implementation phase.
GCPH is seeking an expert public health consulting and facilitation group to
synthesize 18 months of community data and transform high-level
discussions into an actionable, time-bound framework. The primary goal is
to establish formalized partnership agreements and immediate "Quick Wins"
to improve local access to care.
Scope of Work and Methodology
The selected subcontractor will be responsible for leading a 4-month project structured
into three core phases:
Phase 1: Discovery & Synthesis (Months 1)
Conduct an environmental scan and data synthesis by reviewing meeting
minutes and data collected since October 2024.
Conduct 1-on-1 "Barrier Briefings" with key stakeholders across medical, dental,
and behavioral health sectors to pinpoint solvable friction points in the referral
and care system.
Phase 2: Facilitated Goal Formulation (Months 2-3)
Design and facilitate two high-impact "Action Summits" utilizing Modified
Results-Based Accountability (RBA) workshops.
Lead a Prioritization Matrix session to rank community needs by "Ease of
Implementation" versus "Community Impact".
Facilitate the formation of dedicated workgroups focused on the top three "Quick
Win" projects (e.g., a standardized universal referral form or bilingual resource
directory).

Phase 3: Execution & Deliverables (Months 3-4)
Draft an implementation roadmap detailing 1-year and 3-year objectives.
Facilitate the finalization and signing of Memorandums of Understanding (MOUs)
or "Attainable Action Compacts" between providers to ensure clear accountability
for specific access-to-care improvements.
Key Deliverables
The subcontractor is expected to produce the following tangibles:
1. Access to Care Strategic Framework: A board-ready blueprint that includes
specific, data-driven "Quick Wins" and at least three Key Performance Indicators
(KPIs) derived from the local CHA.
2. Formalized MOUs: Signed partnership agreements between local providers
codifying new action items (such as shared sliding-scale protocols or referral
pathways).
3. Coalition Toolkit: A sustainability handover toolkit designed to equip GCPH staff
to independently lead future meetings and track KPIs without requiring ongoing
external facilitation.
GCPH Support and In-Kind Contributions
GCPH will provide in-kind contributions to support the subcontractor,
ensuring 100% of the $50,000 budget is dedicated to the consultant's
professional services and data synthesis. GCPH will provide an internal
Project Manager, administrative support, meeting spaces, and access to all
existing data sets.
Performance and Evaluation Metrics
The subcontractor's success will be evaluated using internal benchmarks and outcome
evaluations established for the grant:
Stakeholder Engagement: Achieving and maintaining 75% consistent
attendance from key organizational leaders at all facilitated summits.
Feedback: Conducting a 3-question "Partnership Survey" after each Action
Summit to measure stakeholder buy-in and goal clarity.

Timeliness: 100% completion of the Strategic Framework by the end of Month 4.
Adoption: Achieving majority consensus from the provider coalition (via MOUs)
and formal adoption of the final plan by the GCPH Board.
