Pennsylvania on November 3, 2026, at 1:30 PM. Attendance, although not mandatory, is strongly encouraged.

SECTION B: INSTRUCTIONS TO BIDDERS
1. Use of Bid Forms
(cid:127) All bids must be submitted on the provided forms and shall be subject to all requirements of these
Instructions.
(cid:127) One of each of the following forms must be submitted as part of the complete bid:
1) The Bid Form & Collection Matrix (Completed and Signed)
2) Non-Collusion Affidavit of Prime Bidder (Signed and Notarized)
3) Statement of Bidder's Qualifications (Completed, Signed, Notarized)
(cid:127) Bid documents shall be sealed in an envelope clearly labeled: "Bid Documents for Comprehensive Waste
Collection & Recycling Services," including the name of the bidder and the date/time of the bid opening.
2. Interpretations
No oral interpretation will be made to any bidder as to the meaning of the Contract Documents. Every request
for interpretation must be made in writing to smereness@dauphinhousing.org. Any inquiry received ten or more
days prior to the date fixed for opening of bids will be given consideration.
3. Inspection of Site
Each bidder should visit all five site locations covered in this contract to fully acquaint themselves with existing
conditions, facilities, difficulties, and restrictions attending the performance of the Contract.
4. Non-Collusion Affidavit
Each person submitting a bid shall execute a non-collusion affidavit in the form provided herein. This affidavit
must be attached to the bid.
5. Statement of Bidder's Qualifications
Each bidder is required to submit a "Statement of Bidder Qualifications" regarding previous experience,
business organization, and financial resources.
6. Time for Receiving Bids / Opening / Withdrawal
(cid:127) Bids received after 12:00 Noon on November 24, 2026, will not be considered.
(cid:127) Bids will be publicly opened and read aloud at 1:00 P.M. on the bid due date.
(cid:127) Bids may be withdrawn on written request received prior to the time fixed for opening.
7. Award of Contract
The contract will be awarded to the responsible bidder submitting the lowest responsive proposal offering a
firm, fixed monthly rate complying best with the conditions of the Invitation for Bids. No dynamic surcharges,
fuel adjustments, or supplemental fees will be authorized during the contract term.

SECTION C: BID FORM & COLLECTION MATRIX
The Undersigned, having familiarized themselves with the existing conditions affecting the cost of the work and
the Scope of Work documents, hereby proposes to furnish all supervision, technical personnel, labor, materials,
machinery, tools, equipment, containers, and transportation services required to execute comprehensive waste
removal and recycling services for a continuous term of Three (3) Years at the following fixed monthly rates:
1. Service Reliability & Standards
(cid:127) Units exhibiting persistent corrosion, defective lids, or broken structural hinges must be replaced or fully
repaired within forty-eight (48) business hours of direct notification.
(cid:127) Missed scheduled collection stops must be fully remediated with off-schedule recovery deployments within
twenty-four (24) business hours at zero ancillary cost to HACD.
(cid:127) Complete monthly transparency reports quantifying exact weights or systemic volumes collected per
property line item must be delivered to the HACD Procurement Officer on a rolling monthly basis.
