Summary

Washington County Public Health (WCPH) is seeking proposals from eligible organizations to expand access to mobile healthcare services and social supports in rural Washington County communities. This project supports implementation of the Washington County Community Health Improvement Plan (CHIP).


Funding for this opportunity is provided through the Oregon Health Authority (OHA) Rural Health Transformation Program (RHTP). OHA receives this funding from a federal initiative administered by the Centers for Medicare & Medicaid Services (CMS). Organizations funded through this Request for Proposal (RFP) will work in partnership with WCPH to implement approved activities that support the goals of this project. This funding opportunity supports community-based approaches that:

  • Expand existing mobile healthcare services in rural communities
  • Improve connections to social supports and community resources

Organizations may apply under one or both priority areas described in this RFP.



Background

Washington County encompasses urban, suburban, and rural communities. Rural areas across the county are characterized by small towns, farms, and incorporated and unincorporated communities. Residents in these communities may experience limited access to healthcare services, transportation, and other supports that contribute to overall health and wellbeing.

WCPH works alongside community partners to support the health and wellbeing of residents throughout the county, including rural communities. Several rural communities in Washington County are designated as Medically Underserved Populations (MUPs) by the U.S. Health Resources and Services Administration (HRSA), a designation used to identify geographic areas experiencing shortages of primary care services.
