Description:

The Napa County Health and Human Services Agency desires to solicit qualified proposals to provide Youth Partner services, one (1) youth partner for youth in foster care or formerly in foster care. The youth partner will provide ongoing support to ILP eligible youth ages 14 ½ to 21. This support will include peer support during Child Family Team Meetings, court hearings and family visitation as requested. In addition, the youth partner will be available to support youth in meeting the services objectives and goals outlined in the transitional living plan to accomplish education, training and employment goals. The Youth Partner will also provide consultation to policy and procedure development and program changes as a subject matter expert with “lived experience” in the Child Welfare system.

The Youth Partner shall work in partnership with Napa County HHSA- Child Welfare Services and will contribute the voice of a youth with lived experience in the foster care system. The Youth Partner will participate in Child and Family Team Meetings, provide mobile response as needed for Family Urgent Response System (FURS) calls and will provide input on the development of Napa County HHSA - Child Welfare Policies and Procedures. The Youth Partner will work to connect transitional age youth, to the resources and services they need in order to achieve academic, employment, and self-sufficiency success through outreach, resource navigation, and follow-up while developing a peer supportive relationship. HHSA – Child Welfare Services anticipates contracting with the successful applicant for a term of one year with at least two additional automatic renewals for a total of three years.

CURRENT PRACTICES
Currently, Napa County Health and Human Services Child Welfare Services Division offers Youth
Partner services through a contract with a non-profit agency located in Napa. The Youth Partner
contributes to Independent Living Program (ILP), which provides services for youth in foster
care or formerly in foster care. ILP and services provides foster youth with greater self-
sufficiency prior to leaving the foster care system by providing independent living skills
assessments, training and case management services to assist in the transition to a successful,
independent adulthood. Examples of skills that shall be taught are home management skills,
such as locating appropriate housing, finding and maintaining employment, cooking, first aid,
money management, substance abuse prevention and preventative health activities, such as
smoking avoidance, nutrition education and pregnancy prevention.
Referrals are submitted to the contractor by Child Welfare Services (CWS) of Health and Human
Services Agency and the Napa County Juvenile Probation Department.

SCOPE OF WORK
The successful respondent will be expected to perform services according to the following scope
of work:
CONTRACTOR shall provide the services and support for youth who have reached out through the
Family Urgent Response System (FURS) mobile response for ongoing support.
CONTRACTOR will provide youth incentives up to a minimum of $1,500 for client participation in
activities.
CONTRACTOR shall participate in Child and Family Team Meetings and Life Conferences when
invited.
CONTRACTOR shall participate on County Work Groups/Policy & Procedure Development.
CONTRACTOR shall participate in facilitated focus groups and committees to offer a youth perspective
and formulate recommendations that will guide County practice and formulate recommendations
that will guide County practice and programming. (Examples of work would include Family First
Prevention Services Act, County Self-Assessment/System Improvement Plan and Commercially
Sexual Exploited Children workgroups).
CONTRACTOR shall provide Outreach and Resource Navigation for foster youth, social
workers/probation officers and caregivers.
CONTRACTOR shall lead the recruitment, screening, interviewing, and selection of the Youth Partner
candidates.
CONTRACTOR shall train the Youth Partner on role, basic job skills, soft skill development, effective
communication with supervisor, peer resource navigation service role and activities, specific suite of
resources for Transition Age Youth, and secondary trauma and self-care.
CONTRACTOR shall provide Youth Partner with computer and technology access.
CONTRACTOR in partnership with Health and Human Services - Child Welfares Division shall
perform supervision of the Youth Partner, ensuring their performance meets the expectations of the
Department. The Youth Partner shall maintain scheduled office hours at the Child Welfare Services
office location at a minimum of sixteen hours a week which may include some weekend and evening
work, two eight hour days per week.
CONTRACTOR in partnership with Napa County CWS, shall plan and ensure the Youth Partner
engaged in a variety of activities, supporting the needs of the Department and youth in Napa
County, including participation in HHSA- CWS unit meetings, HHSA-CWS All Staff meetings and
applicable trainings offered to HHSA-CWS staff.
CONTRACTOR shall provide trainings and resources to assist the Youth Partner with their advocacy
work, professional development and personal growth.
CONTRACTOR shall provide administrative structure and support for overall program
implementation, including providing a computer, cell phone and other office supplies, as needed.
CONTRACTOR shall support professional development opportunities for the Youth Partner which
include but are not limited to conferences, trainings, workshops and panel participation as deemed
appropriate by the HHSA- CWS.
OUTCOMES
Youth contacting the FURS mobile response will be supported by a youth partner, as needed, which
will ensure engagement and support for the youth’s voice to be heard.
Youth participating in a FURS mobile response will maintain in their placement.
Youth participating a FURS mobile response will receive support services timely and according to the
mobile response action plan developed with youth input.
