Introduction
Nassau Health Care Corporation (“NHCC”) is seeking proposals from qualified software vendors to
provide a comprehensive, evidence-based Case Management Decision Support Software Tool to assist
Case Management, Utilization Review, and Physician Advisor teams in determining appropriate patient
status. The software must support compliant workflows, integration with NHCC systems, and
regulatory alignment.
Proposal Requirements
Proposals must comply with all instructions outlined in this RFP. Submissions that are incomplete
or fail to meet the specified requirements will be rejected without review. Any contract awarded as
a result of this RFP will supersede all prior contracts, bids, or Group Purchasing Organization
(GPO) agreements related to the specified services.

I. GENERAL INFORMATION
A. Background
Nassau Health Care Corporation (“NHCC”), also known as the NuHealth System, is a New York
State public benefit corporation established under the Public Authorities Law. NHCC operates:
• Nassau University Medical Center (NHCC): A 530-bed tertiary care teaching hospital.
• A. Holly Patterson Extended Care Facility (AHP): A 589-bed skilled nursing facility.
• Community Health Practices (CHP): Operated in partnership with Long Island FQHC, Inc.,
a non-profit, consumer-driven organization serving the region’s most vulnerable
populations.
NHCC is committed to delivering high-quality care, advancing medical education, and serving as
the safety-net health system for Nassau County residents.
Nassau University Medical Center (NHCC): Founded in 1935, NHCC is the County’s primary
source of advanced medical care. Its 19-story tower houses the region’s premier Level I Trauma
Center and serves as a hub for medical education through affiliations with the New York Institute of
Technology College of Osteopathic Medicine, the New York College of Pediatric Medicine, the
American University of the Caribbean School of Medicine, and the Donald and Barbara Zucker
School of Medicine at Hofstra/Northwell.
A. Holly Patterson Extended Care Facility (AHP): Nationally recognized as a model for skilled
nursing services, AHP provides innovative, patient-centered care that addresses residents’ physical,
emotional, and social needs in a supportive, healing environment.
Community Health Practices: NHCC’s community-based centers deliver comprehensive,
accessible, and preventive care to underserved communities across Long Island, ensuring every
resident has access to a trusted “medical home.”
B. Proposal Overview
Nassau Health Care Corporation (“NHCC”) is soliciting proposals from qualified Proposers to
provide an evidence-based Case Management Decision Support Software Tool that supports
appropriate patient status determinations, utilization review workflows, documentation, reporting,
and payer audit readiness. All Proposers must demonstrate experience providing robust, clinically
validated software tools that support inpatient, observation, and outpatient level-of-care
determinations across diverse patient populations.
Proposals must align with the requirements set forth in this Request for Proposal (“RFP”). NHCC
reserves the right to reject any submission that is incomplete or fails to comply with the
specifications of this RFP. All communications and submissions must reference RFP #012-2025 to
ensure proper handling.

II. SPECIFICATIONS OF THE SERVICES
A. Scope of Work / Requested Services
Nassau Health Care Corporation (“NHCC”) seeks to procure evidence‑based, software‑based
clinical decision support tools used by the Case Management and Utilization Review departments to
determine appropriate patient status. The software must assist with evaluating inpatient,
observation, and outpatient level‑of‑care determinations and must support regulatory compliance,
payer audit defensibility, and standardized documentation.
The requested software must:
• Provide clinical criteria for adult, pediatric, behavioral health, and emergency department
populations.
• Supply evidence‑based, nationally recognized guidelines updated at least annually.
• Deliver a searchable, intuitive software interface.
• Support documentation and audit trails for UR and Physician Advisor review.
• Provide reporting functionality and administrative dashboards.
• Offer licensing for all required NHCC staff.
B. Scope of Services

The solution must:
• Support Single Sign‑On (SSO)
• Provide API or HL7 interoperability
• Meet HIPAA security standards
• Integrate with NHCC systems, including SCM, Eagle, Aprima, CarePort, and
revenue cycle platforms
4. Implementation & Training Requirements
Proposers must outline:
• Implementation timeline and milestones
• Staff training plan

5. Support, Maintenance & Update Requirements
The proposal must describe:
• Annual criteria update process

• System uptime ≥ 99.9%
• Criteria updates within 30 days of release
• Help desk response within 1 business day
• High‑priority issue resolution within 72 hours
• ≥ 90% user satisfaction
7. Compliance Requirements
Vendor must demonstrate compliance with:
• HIPAA standards
• CMS Conditions of Participation
• NYS confidentiality laws
• Joint Commission requirements
• NHCC internal data protection policies
III. Deliverables
The selected Proposer shall provide the following deliverables throughout the engagement:

• System availability ≥ 99.9%
• Help desk response within 1 business day
• Critical issue resolution within 72 hours
• Criteria updates deployed within 30 days of official release
• User satisfaction ≥ 90% measured annually
IV. Evaluation Criteria
NHCC will evaluate all responsive proposals in accordance with the criteria outlined below.
Proposers are advised that award will not be based solely on price; rather, it will reflect the overall
best value to the organization, taking into consideration the technical, operational, and financial
merits of the proposal.
Each proposal will be reviewed using the following evaluation factors:
1. Clinical and Technical Merit of the Proposed Software
This includes an assessment of the platform’s ability to meet NHCC’s operational and
regulatory needs, with emphasis on:
• Evidence-based clinical criteria content
• Breadth of adult, pediatric, ED, surgical and behavioral health criteria
• Accuracy and clarity of decision support outputs
• Frequency and reliability of criteria updates
• Compliance with applicable federal and state regulations
• System performance, uptime, and overall stability
