Professional Services Requested: Cancer Screening for The West Orange Fire Department

The Township of West Orange, is seeking the services of a Cancer Screening for The West Orange Fire Department. The proposal package can be obtained in person at the Township of West Orange, 66 Main St., Purchasing Room 111, West Orange, NJ 07052, during regular business hours M-F 8:30 AM – 4:30 PM or online at www.westorange.org . Submissions by Corporations, Partnerships and sole proprietors must include a completed Disclosure of Ownership (N.J.S.A. 52:25-24.2), a valid NJ Business Registration Certificate P.L. 2004, c57, and must comply with the Equal Employment Opportunity requirements of N.J.S.A. 10:5-31 et seq. and N.J.A.C. 17:27. Additional Pay to Play Requirements Contractors are advised of the responsibility to file an annual disclosure statement on political contributions with the New Jersey Election Enforcement Commission, pursuant to N.J.S.A. 19:44-20.13(P.L. 2005, c, 271, s.3), if the contractor receives contracts in excess of $50,000 in the aggregate from public entities during the calendar year. The Township of West Orange (Township) is issuing this Request for Qualifications (RFQ) for qualified
individual to provide Consultant services of a specialized nature for the Township as directed by the
appropriate official within the Township for a contract period ending 12.31.2026. All candidates are
required to comply with N.J.S.A. 10:5-31 et seq. and N.J.A.C. 17:27 et seq. as amended (Affirmative
Action). The successful professional must have significant experience in representing municipal clients
or other local governments and public agencies in the State of New Jersey. The Township of West
Orange reserves the right to pre-qualify and/or contract with an individual.
N.J.S.A 52:14-17.29ii(a) requires access to periodic cancer screenings for full-time paid
firefighters not enrolled in the State Health Benefits Plan (SHBP) within three years after the
start of the firefighter's employment and every three years thereafter during the course of
employment. The examination shall include, at a minimum and as applicable, the screening for
eleven types of cancer pursuant to N.J.S.A 52:14-17.29ii(a). The State of New Jersey, through
the Department of the Treasury's Division of Administration, will reimburse eligible public
employers a maximum of $1,250 per firefighter over a three year period towards the costs
associated with providing the required cancer screenings.

SCOPE OF SERVICES
2 Phases
a. Phase 1 - Onsite or in office visit
Physical & Blood draw – Provider review of complete health history, Oral Cancer
Screening & Full Skin Cancer Screening.
Cancer Screening Ultrasound
Script provided for Low Dose Lung CT Screening & appt
scheduled at an offsite location convenient to member
Telehealth appt 1 week out from onsite or in office visit appt
Testing should include, at a minimum, testing and screenings for the following
types of cancer in accordance with N.J.S.A. 52:14-17.29ii(a)(1)-(11):
1. Colon
2. Lung
3. Bladder
4. Oral
5. Thyroid
6. Skin
7. Blood
8. Breast
9. Cervical
10. Testicular
11. Prostate
Phase 2 – 20-minute telehealth results review –Provider consults on patient results
(Ultrasound & comprehensive Blood Work review). If member completed the CT
screening and results are ready the consult will include those as well.
i. All members have access to their complete medical record & results in
a medical system provided by you. Vendor to provide login instructions
for accessing those results to ease follow up.
ii. Provider follow up ensures all members understand their results and if
any follow up is required they are directed appropriately to their primary
care provider.
Additional Services to be included:

a. provides all marketing & scheduling materials.
i. Registration information – Demographics of members (for lab
requisitions), eligibility (active members only and have at least 3 years as
an active-duty firefighter).
Scheduling – Program delivered mostly on duty – provides registrant list to contact of Township
choice to have them coordinate the schedule/times provides coverage for all shifts.
Procurement Process and Schedule
The selection of Qualified Respondents is not subject to the bidding provisions of the Local
Public Contracts Law, N.J.S.A. 40A:11-1 et seq. The selection is subject to the New Jersey
Local Unit Pay to Play Law, N.J.S.A. 19:44A-20.4 et seq. The Township has structured a
procurement process to assure that each Individual is provided an equal opportunity to
submit a Qualification Statement in response to the RFQ. Qualification statements will be
evaluated in accordance with criteria set forth in section 2 of this RFQ, which will be applied
in the same manner to each Qualification Statement received. Respondents agree to at all
times to abide by all requirements of the New Jersey law, including, but not limited to the
aforementioned “Pay to Play” laws, as well as any and all relevant Executive Orders and the
New Jersey Election Law Enforcement Commission disclosure requirements.
Conditions Applicable to RFQ
Upon submission of a Qualification Statement in response to this RFQ, the Respondent acknowledges
and consents to the following conditions:
This RFQ does not commit the Township of West Orange to issue a Request for Proposal.
The Township reserves the right to exercise its judgment to weigh the relative merit and
deficiencies, if any, in all responses and commits. The Township reserves the right to reject for
any reason, all responses and components thereof and eliminate any and all Respondents
submitting this RFQ from consideration.
The Township reserves the right (in its sole judgment) to reject any Respondent that submits
incomplete responses to this RFQ, or a Qualification Statement that is not responsive to this
RFQ.
The Township reserves the right, without prior notice, to supplement, amend, or otherwise
modify this RFQ, or otherwise request additional information.
All Qualification Statements shall become the property of the Township and will not be
returned.
All Qualification Statements will be made available to the public at the appropriate time, as
determined by the Township (in the exercise of its sole discretion) in accordance with law.
The Township may request Respondents to send representatives to the Township for
interviews.
