II. Scope of Services
A. Background
The mission of the Division of Public Health is to protect and enhance the health of the
people of Delaware. The Division accomplishes its mission by:
* working together with others;
* addressing issues that affect the health of Delawareans;
* keeping track of the State's health;
* promoting positive lifestyles;
* responding to critical health issues and disasters;
* promoting the availability of health services.
The accomplishment of this mission will facilitate the Division in realizing its vision of
creating an environment in which people in Delaware can reach their full potential for a
healthy life.
Project Background
The Delaware Division of Public Health (DPH) administers several preventive health
programs, including Screening for Life (SFL), designed to provide cancer screening and
diagnostic services for eligible Delaware residents and Well-Integrated Screening and
Education for WOMen Across the Nation (WISEWOMAN) aimed at reducing
cardiovascular disease and stroke risk factors among women. To ensure timely,
coordinated, and client-centered access to these services, DPH seeks to engage Patient
Navigators who will guide clients through the cancer screening and diagnostic process
and ensure clients, aged 35 to 64, receive an integrated office visit via the WISEWOMAN
program.
The Delaware Screening for Life (SFL) Program serves as the implementation program
for the National Breast and Cervical Cancer Early Detection Program (NBCCEDP).
Established in 1997, the SFL program offers dual eligibility determination and enrollment
into both the SFL Program and/or the Health Care Connection (HCC) program. It also
provides integrated office visits for the WISEWOMAN program during breast or cervical
screening appointments.
SFL provides preventive cancer screening services to eligible clients, including lung,
colorectal, prostate, breast, and cervical cancer screenings. The program utilizes a
combination of state and federal funding to support these services. In addition to screening
services, SFL offers comprehensive care coordination to help mitigate barriers to
screening, such as transportation, documentation, or follow-up scheduling. The
WISEWOMAN program is a collaborative program with Screening for Life that engages
4

STATE OF DELAWARE
Division of Public Health
women during their breast or cervical cancer screening appointment and offers specific
blood tests to measure blood sugar, cholesterol, and other indicators for cardiovascular
disease or stroke risk.
Patient Navigators play a vital role in improving patient outcomes by addressing barriers
to care, introducing and explaining the cancer screening and diagnostic process, ensuring
continuity, and facilitating communication between patients, health promotion advocates,
healthcare providers, and program administrators. Patient Navigators will also link
Delaware residents to additional healthcare service needs and social services resources.
Patient Navigators will work with Health Promotion Advocates (HPAs), the primary point
of contact for clients seeking enrollment into preventive health programs such as
Screening for Life and Health Care Connection, to facilitate access to essential cancer
screenings, and link to additional healthcare and social services resources.
HPAs are embedded within healthcare and community service facilities to ensure face-to-
face engagement with clients, streamline application processes, and facilitate navigation
through SFL and HCC program eligibility, enrollment, and follow-up. Their efforts are
essential to improving early detection rates and addressing health disparities across
Delaware. HPAs may coordinate client referrals with Patient Navigators.
B. Project Goals
The primary goals of this project are to:
1. Ensure that all clients eligible for Screening for Life (SFL) and non-SFL
clients receive effective, consistent, and compassionate navigation
throughout their screening or diagnostic journey through to final diagnosis.
If cancer is diagnosed, make appropriate referral for additional resources
such as the Delaware Cancer Treatment Program, survivorship programs,
or other needs.
2. Improve access to cancer screening and diagnostic services by
eliminating barriers such as transportation, language, and other social
determinants of health.
3. Ensure that clients, aged 35 to 64, receive a WISEWOMAN integrated
office visit during their breast or cervical cancer screening appointment.
4. Enhance coordination between hospital systems, the Division of Public
Health, and SFL program staff to streamline data collection, case
management, and follow-up processes
5. Maintain accurate, timely, and complete documentation of all client
navigation activities, screenings, and outcomes in data systems to be
determined by SFL and DPH (e.g. SFL database and Smartsheets).
6. Support overall DPH objectives of improving cancer prevention, early
detection, and reduction of incidences of chronic disease, cardiovascular
disease, and obesity, and promoting equitable health outcomes across
Delaware
