California Home Visiting Programs (CHVP)

The County of Yolo is requesting proposals from qualified vendors to deliver comprehensive California Home Visiting Program(s) per the scope of work outlined in this RFP. Proposers may submit responses to this RFP, offering to deliver one (1) of the following evidence-based home visiting programs:

No subcontractors are permitted.

It is anticipated that a single contract will be awarded a maximum total term of five (5) years. Funding will come from a combination of federal and state funds. The final budget allocations available have not been finalized yet; however, it is tentatively estimated that any contract awarded will be for a total funding level of approximately $875,000 a year, broken down as follows:

Actual allocations from each funding source will be finalized as part of any contract negotiations with the best evaluated vendor. Proposers who submit a response to this RFP must have the ability to meet the requirements, including the terms and conditions contained in this RFP.

There will be an optional pre-proposal conference held at this time.
