. DESCRIPTION: This MBE SET-ASIDE SOLICITATION is to obtain a contractor to provide Human Milk Storage Guidelines Magnets - English and Spanish for use by the Ohio Department of Health.

2. QUANTITY: 14,000 (EXACT QUANTITY) HMS GUIDELINES MAGNETS (TWO VERSIONS)
VERSION A: 12,000 Human Milk Storage Guidelines Magnets – English (Exact quantity).
VERSION B: 2,000 Human Milk Storage Guidelines Magnets – Spanish (Exact quantity)

3. UNIT PRICE AWARD: Bidder shall not insert a unit cost more than 3 digits after the decimal point. Digit(s) beyond 3, after the decimal point shall be dropped by DOH and not used in evaluation and any subsequent award. To determine the low lot total price of the quick quote, the state will multiply the estimated usage of each item by its corresponding unit price and add the totals together. Failure to bid all items will disqualify your bid.

4. SPECIFICATIONS: Human Milk Storage Guidelines 7” x 4” Magnets, 20 mil white matte / smooth finish magnet material, prints one side, four color process with full bleeds.

B. STOCK: 20 mil white matte / smooth finish magnet material
C. PRESSWORK: Shall print one side, four color process with full bleeds. D. INK: Four Color Process.
E. BINDERY: Trim to size 7” x 4”
F. FINISHED TRIM SIZE: 7” x 4”
G. SUPPLIED TO VENDOR: PDF artwork plus distribution list WILL BE FURNISHED TO THE SELECTED VENDOR. Return all state supplied materials to the proof to person.
H. PROOF: A high resolution exact color match PDF proof of both version English and Spanish shall be emailed to morgane.puleikis@odh.ohio.gov, Colleen St.Cyr at colleen.st.cyr@odh.ohio.gov, Scott Ciaccia at scott.ciaccia@odh.ohio.gov

I. PACKAGING: Carton pack, label with title “Human Milk Storage Guideline Magnets” and ship both items of various quantities to 71 different locations as noted in the Human Milk Storage distribution list.

J. ADDITIONAL SPECIFICATIONS: Distribution list attached to this solicitation.

5. DELIVERY: REQUESTED, JUNE 22, 2026 OR SOONER Ship both items of various quantities to 71 different locations as noted in the provided Human Milk Storage distribution list. Awarded bidder shall provide required proof of delivery for all deliveries.
