TO ALL PROSPECTIVE BIDDERS:

The above described SEALED BID has been modified to reflect the following changes.

1. The Bid Proposal Form, to be provided along with the Quotation/Bid Response Form

(reference Section IV), is included as Attachment A.

2. The March 1958 Supplementary Office and Court Office, January 2010 Building

Modernization, and April 2010 Elevator Modernization Drawings for 157 Poplar are

incorporated as Appendix C, D, and E respectively for information purposes only.

Vendor is responsible for verification of all spaces, space types, and space FAS

requirements and provide same.

All other requirements of the SEALED BID remain unchanged.

examined the Place of The Work and all matters referred to in the Instructions to Bidders

and the Contract Documents prepared Shelby County Government for the above-

mentioned project, we, the undersigned, hereby offer to enter into a Contract to perform the

2. ACCEPTANCE

This offer shall be open to acceptance and is irrevocable for one hundred twenty days from

the bid closing date. If this bid is accepted by the Owner within the time period stated above,

we will:

 Execute the Agreement within seven days of receipt of Notice of Award. In the form

described in Supplementary Conditions.

 Commence work within ten (10) days after written Notice to Proceed. If this bid is accepted

within the time stated and we fail to commence the Work or we fail to provide the required

bonds, the security deposit shall be forfeited as damages to the Owner by reason of our

failure, limited in amount to the lesser of the face value of the security deposit or the

difference between this bid and the bid upon which a Contract is signed. In the event our

bid is not accepted within the time stated above, the required security deposit will be

returned to the undersigned, in accordance with the provisions of the Instructions to

Bidders; unless a mutually satisfactory arrangement is made for its retention and validity for

an extended period of time.

3. CONTRACT TIME

If this Bid is accepted, we will: Complete the Work in 180 calendar days from Notice to

Proceed. CONTRACTOR agrees to provide COUNTY an amount equal to 500 ($) Dollars

per calendar day for liquidated damages for each consecutive calendar day required for the

completion of the contract beyond the time stipulated.
