BIOMED A108 RENOVATION

Project Description: The project includes but is not limited to the demolition of existing carpet and resilient base board and replacing with new carpet tile and resilient base board and removal of, preparation for and application of paint. Work also includes abatement, handling and disposal of hazardous materials.

Provide all labor, tools, materials, equipment and insurance necessary to carry out the safe removal, proper handling, cleanup, and air monitoring, transportation and safe disposal of asbestos-containing material (ACM) in compliance with all applicable federal, state and local laws and regulations concerning asbestos, including all incidental and pertinent operations, from the areas affected by the planned improvements project at the University of Hawaii at Manoa, BioMedical Science Building Room A108 demo, install replacement carpet tiles and baseboards as well as properly dispose of (off site) existing carpet and baseboards.

Remove/dispose of existing carpet, thresholds and baseboards off campus.

Prep existing flooring for laying of carpet tiles and baseboards for repalacement of baseboards per recommended manufacturer installation guidelines.

Lay carpet tiles and baseboards (See Attached for specifications for carpet tiles and baseboards) per manufacturer instructions. Carpet Tiles to be laid in quarter turn pattern.

Provide all labor, tools, materials, equipment and insurance necessary to carry out the safe removal, proper handling, cleanup, air monitoring, transportation and disposal of paint that contains lead in compliance with all applicable federal, state and local laws and regulations, including all incidental and pertinent operations, from the areas affected by the planning improvements project a the University of Hawaii at Manoa BioMedical Scheince BUilding Room A108.

Special Instructions

1. All Offerors shall review the attached documents before submitting a quotation.

2. Attached to this RFQ are the following documents:

a. Mandatory terms and Conditions which includes the Notice to Offerors, Quotation Information, Special Provisions, and General Provisions (www.hawaii.edu/wp/wp-content/uploads/docs/facilities-contracts/GenProv-Construction-9-2013.pdf).

b. Drawings and Technical Specifications

3. A site inspection will be held and will be only opportunity to review the areas of work, refer to attached Notice to Offerors.

- A site inspection has been scheduled for Friday, 13 March 2026 at 11:00 a.m. at 1960 East West Road, Courtyard A, Room A108, Honolulu, HI 96822. Please see attachment for specification of required carpet tiles and baseboard material (or approved equivalent). Contact Carlton Ho at carlho@hawaii.edu to confirm attendance by COB Thursday, 12 March 2026 at 4:00 p.m.

4. In the event that the offeror is awarded this RFQ and its remittance address differs from the offerer\\\'s order address, indicate the remittance address on the Note to Buyer.

5. Offer pricing shall be held for minim of 90 days

6. Offeror to submit the following upon receipt of Superquote Award Notice:

-Hawaii Compliance Express “Certificate of Vendor Compliance”

-Listing of Joint Contractor and/or Subcontractors OPRPM Form 54

-Copy of Valid State of Hawaii Contractor\\\'s License B

-Certification of Compliance with HRS396-18, Safety and Health Programs (for proposals in excess of $100,000)
(Documents shall be scanned and emailed to TRPO as a pdf file).

Failure to submit these documents to the TRPO within 7 days of award notification by TRPO to awa4rd Offerer will result in selection of next lowest bidder.
