Plans and specifications for the “West Mary Street Road and Drainage

Improvements” are open to inspection at City Hall, Dublin, Georgia or may be

obtained from the City of Dublin, Purchasing Department, P.O. Box 690, or 702 East

Madison Street, Dublin, Georgia.

Please contact David Sawyer by email at sawyerd@dublinga.org to obtain the plans

and specifications.

The cost for a set of plans and specifications is $150.00 by a check made payable to

the City of Dublin.

Be sure to submit this signed form with your bid to acknowledge your receipt of

the addendum. Failure to return signed addendum may result in disqualification

of bid.

I have read the above change to the original bid and will submit my bid
