Solicitation for Nursing Services
The Town of Holliston is seeking a Registered Nurse or Registered Nursing Services Company to
provide services, as outlined below, for the Town of Holliston’s Board of Health as set forth
herein. Interested applicants should submit a Resume, Cover letter and fee schedule to Scott
Moles at moless@holliston.k12.ma.us any time prior to 4:00PM October 1, 2026 to be
considered. The Town will contact qualified candidates for interview by October 30, 2026.

Duties and Criteria are as follows:
A. Duties: A Registered Nurse assigned as a primary contact shall perform services
necessary to the delivery of high quality public health nursing care, as requested by the
Town. These services can include the following:
1. Communicable Disease Control:
a. Completion of epidemiological investigations, for all State-required
reportable communicable diseases:
i. Completion of all surveillance forms, follow-up visits, instruction
and /or referral of patients to other sources of assistance
ii. Notification to the Board of Health if a case is a food handler, child
care or health care worker, or other high risk worker; and all
associated required record keeping
iii. Follow-up of all reportable diseases as needed
b. MAVEN: initiate communicable disease investigation and follow up using
Massachusetts Department of Public Health MAVEN, of faxed or phoned
in referrals, or referrals from the Town, as appropriate, in keeping with
the need for timely investigation and follow-up.
2. Adult Health Services: as requested by the Town:
a. home visits as necessary to assess needs, suggest appropriate action, and
referral to other sources
b. visits to newly-arrived immigrant families, to discuss health care services
3. Flu Clinics: as needed assist in arrangements for staffing, staffing support, and
assistance with securing supplies for Town sponsored, Board of Health scheduled
clinics.
4. Maternal/Child health: mother-infant assessment visits; visits to high risk parents
and children, as requested by the Town, in collaboration with physicians and/or
other health care providers.
B. Additional services may be requested by the Town to provide training services
including, but not limited to CPR, first aid, defibrillator training.
C. Hours: The Town will call in requests for services during regular day-time office hours:
7:30 AM – 4:30 PM, Monday through Friday, excluding holidays.

D. Reports: The applicant will submit a monthly report of services rendered, including all
copies of all surveillance reports by the 15th of the month following service. The
applicant will maintain records and submit appropriate reports as required by the State.
E. Fees: The Town will negotiate a rate of payment for an hourly rate per home visit,
telephone consultation and/or coordination for all Town requested or State required
communicable disease case work. Payment for nursing attendance at flu clinics or other
Town activities will be at the negotiated rate.
F. Terms of Agreement: The applicant who is awarded a contract with the Town will be
expected to enter into a one-year contract with the Town in substantially the form
provided. The town may renew the contract at the end of the term for up to two
additional years. Either party may terminate the Agreement between the parties
provided sixty (60) days advance notice is given to the other party in writing.
G. Independent Contractor: The applicant or corporation awarded a contract with the
Town will be considered an independent contractor and not an employee of the Town
for purposes of workers’ compensation or benefits. Contractor will be required to
maintain profession liability insurance and provide a W-9 form to the Town.
