The purpose of this Request for Quotes (RFQ) is to enter into a contract or purchase order with the responsive and responsible vendor that submits the lowest cost to provide three different MedVision Manikins.

We need the following features included with Arthur:
Arthur Lifelike Pediatric Patient Manikin; Instructor
Laptop; Patient Monitor; Software Suite (GUI Instructor SW, Patient
Monitor SW, Debriefing SW, Scenario Constructor SW, EKG Editor
SW) - software licenses for life; Wi-Fi Router; Simulator Battery and
Charger; Pulse Oximeter with Adapter; “BlueScope” Auscultation
Device and Charger; Auscultation Sounds Library; BP Cuff;
Medicine Kit with Syringes and RFID Writer and Tags; IV port;
Lubricant (Spray); IO Consumables; Neck Consumables;
Replaceable Silicone Genitals (M/F); Genitals Replacement Key;
Shorts and Shirt.
We need the following features included with Leonardo:

Wound; Bleeding Leg Wound; Replaceable Silicone Genitals (M/F);
Genitals Replacement Key; Shorts.
Full installation and tutorial.
We need the following features included with Matt:
Torso with lifelike skin; Library of real clinical sounds: 41 heart
sounds, 18 lung sounds, 14 bowel sounds; 20 LED auscultation
point indicators; sound to speaker capabilities; anterior/posterior
auscultation points (5 heart, 11 lung, 4 bowel); pivoting base.
Includes controlling computer, software and license, Bluescope
(compatible with real stethoscopes) and Bluescope charger.
Payment Terms
Net 30.
The College will pay with a purchase order or by credit card. No credit card processing fees are allowed (must be included in bid pricing). The College will make payment at the satisfaction of the project manager and the total completion of the project. Any supplier requiring money down, can be eliminated from the bid list, or will be required to provide a performance and payment bonds in the amount of the bid. Pricing of the bid must include the price of the bond(s), and is considered part of the bid total price.

Evaluation of Quotes: Each quote received shall be evaluated for responsiveness in as outlined in the Utah Procurement Code. This RFQ shall be evaluated by the objective criteria described in this RFQ. Quotes submitted must comply with the specification identified in this RFQ.
Multiple or alternate quotes will not be accepted. If a vendor submits multiple or alternate quotes, Snow College will only accept the bidder's primary bid and will not accept any other bids constituting multiple or alternate bids.
Snow College Purchasing may solicit other quotes via email, phone, or other methods as permitted by the Utah Procurement Code or applicable Administrative Rules.
