Bennett Therapy Room Equipment

The purpose of this solicitation is to establish an Agreement with FDVA for
the Emory L. Bennett State Veterans’ Nursing Home located at 1920 Mason Avenue, Daytona Beach,
FL 32117 (Volusia County). The successful Contractor shall provide labor, materials, for the removal of
existing equipment and the installation and assembly of new equipment listed in Section III “Statement
of Work”. FDVA invites interested Contractors to submit a bid in accordance with this solicitation.
Contractor will provide the removal of old therapy equipment which will be retained by Bennett SVNH.
Contractor will provide new therapy equipment and furniture listed in Section III “Statement of Work”.
Contractor will install and assemble all new therapy equipment and furniture unless otherwise specified
in the scope of work. It is the sole responsibility of the vendor to confirm with the Contract Manager prior
to beginning work. Vendor will make sure all equipment is in working order. Vendor will dispose of all
waste. Vendor is responsible for any damages to the property and or therapy equipment and or furniture
during delivery and setup.
For further specific details, see Section III “Statement of Work”. The term of this Contract shall
commence on the date of the Agreement’s full execution, with no renewals. FDVA requires specified
services to be completed to the full satisfaction and acceptance of FDVA and any applicable authorities
having jurisdiction, within ninety (90) calendar days from the date of the Agreement’s full execution
