Stony Brook University Hospital intends to purchase Avantik's Universal Transport Medium NP Kit - 3ml (Prod# GL5080) under its discretionary authority, pursuant to NYS Education Law §355(5)(A), which is up to $125,000, for the above noted commodity/product or service. A sealed bid will not be held. Successful contractor shall be required to complete and submit documents related to Vendor Responsibility, NYS Finance Law 139, NYS Tax Law Section 5-a, and valid/current NYS Workers’ Compensation and Disability Forms.
