2 General
The University of Iowa, located in Iowa City, Iowa (hereafter, "University") desires to obtain written proposals from
qualified suppliers to provide an assessment of Medicare Cost Report Disproportionate Share Activity.
Although the University has listed general criteria that will be used in its final evaluation, the University does not
intend these criteria to limit the Supplier's creativity in preparing a proposal it believes will accomplish the
University's goals. The use of a specific Supplier will be based on the Supplier's performance.
The required documentation of expertise and qualifications outlined in this request is intended to serve primarily as
a general guide for each statement of qualifications, with the minimum requirements listed. Each Supplier is
expected to submit a fully detailed statement of qualifications, which adequately describes the advantages and
benefits the University would realize by selecting the Supplier.

3 Background
The University of Iowa
The University is a broad-based public university of international stature. As of 2022-2023 The University has
31,317 students and is recognized for its academic achievement; leadership in research; and its teaching hospital
The University of Iowa Hospitals and Clinics. As of 2022 The University has an overall budget of $4.4 billion and
employs approximately 19,000 full-time faculty and staff. More information can be found at the Office of the
Registrar.
The University of Iowa Hospital and Clinics
The University of Iowa Hospitals and Clinics serves as the teaching hospital and comprehensive medical center for
the State of Iowa. As part of its mission, UI Hospitals and Clinics, in conjunction with UI Carver College of Medicine
and other health science colleges at The University of Iowa, provides world-class family-centered health care,
extensive medical research, and comprehensive teaching programs for many health care professions. The hospital
employs more than 924 physicians and dentists, and more than 2,445 professional nurses. More than 10,224
additional staff members support all aspects of patient care services. The hospital provides 811 inpatient beds and
more than 1 million patient visits take place in the clinics every year. UI Hospitals and Clinics is consistently ranked
by U.S. News & World Report magazine as one of the nation's best hospitals, with many of its clinical specialties
ranked among the top twenty-five in the country.
University of Iowa Health Care Medical Center Downtown
On January 31, 2024, University of Iowa Health Care acquired an additional health system which includes a hospital
with a separate 194 inpatient bed license and 27 outpatient clinic locations across Eastern Iowa.

4 Scope of Services
The successful Supplier will work in partnership with the University and will be expected to fulfill the following scope
of services, including but not limited to the following:
The University seeks a qualified firm to analyze inpatient discharges during the hospital’s Cost Report year to
determine if the days associated with each discharge do or do not qualify for Disproportionate Share Hospital (DSH)
days, as well as Social Security Income (SSI) evaluation.
The successful Supplier will work in partnership with the University and will be expected to fulfill the following scope
of services, including but not limited to:
1. Analyzing each discharge for both Medicaid and Medicare eligibility
2. Incorporating state paid claims files
3. Matching all newborns to their mother
4. Performing a labor and delivery analysis
5. Analyze all SSI info and support the University in associated activities related to Medicare Cost Report filings,
updates, and/or audits
The successful Supplier will also be required to:
1. Assess claims for eligibility into the Medicaid Waiver programs in place in Iowa. In addition to the detailed
analysis, the qualified firm will provide all supporting schedules and documents for the Operating and Capital DSH
payment related to reporting Medicare Cost Report Worksheets S-2, lines 24-25 & S-3, Part I, Column 7 (or
replacements/expansions thereof).
2. Generate detailed DSH listings and gather required documentation to support the Cost Report schedules
required to comply with the Fiscal Intermediary Cost Report audits.
3. Support the University in responding to audit questions from the Fiscal Intermediary regarding the DSH listings.
4. Provide an analysis of protest amounts for any issues arising related to DSH payment calculations, including but
not limited to, Medicaid fraction days, Social Security Income (SSI) days and dual eligible days.
5. Assist the University as a partner, or finding a partner, to file any protests with the appropriate government
agencies for review and compensation recovery.
6. Additional related services may also be requested throughout the scope of work.
The successful Supplier will provide an initial analysis prior to the Medicare Cost Report filing deadline, and also
work w/the University to provide an updated analysis for use prior to the Cost Report audit. The University desires
to have a firm in place by July 1, 2024, with a first draft available mid-October 2024 (or as requested by the
University), to support each hospital’s (and associated campuses) FY2024 Medicare Cost Report submissions,
which are due November 30, 2024. Subsequent Cost Report years to follow similar timelines and requests.
The final work product (with refreshed data) will be completed approximately one year later (due date to be
negotiated in confines of current cost report regulations).
The successful Supplier will also analyze all SSI information and will support the University in filing any updates to
each hospital’s prior, current, and future Medicare Cost Report filings for any missed opportunities allowable via
administrative or regulatory guidance or publication. The analysis will include any supporting documentation
required for the filing update and any audit requirements by the Fiscal Intermediary.
5 Scope of Services - Changes
The University reserves the right to change the scope of work as required. Notices of such changes will be made in
writing by the University to the Supplier.

6 Supplier Requirements
Suppliers must clearly demonstrate and provide documentation substantiating that they are compliant in each of the
following areas. The University reserves, solely, the right to reject any submittals if the evidence or references
submitted by such Supplier fails to satisfy the University that said Supplier is properly qualified in any of these
areas. It shall not be the responsibility of the University to request additional information to satisfy these
requirements, if such information is not provided with the submitted response. Failure to respond to any question or
follow the instructions herein may result in disqualification
7 Project Proposal
Provide a proposal detailing the firm’s process to be used in providing the services described herein. The proposal
should clearly indicate any major requirements that cannot be met by the firm and highlight the major features of
the proposal to assist the reader in determining generally how the qualifications of the firm and the proposal meets
and exceeds the requirements proposed by the University.
Please include, at minimum, the following information in your proposal:
1. Identify specific areas where your firm is best equipped to assist. Explain your firm’s past experiences that
demonstrate the firm’s expertise in providing institutions similar to the University of Iowa with similar services.
2. Explain the process the firm uses for projects of this nature. How does the firm intend to manage the relationship
with the University throughout the life of the contract?
3. Provide the following information in your proposal regarding your firm’s qualifications:
a. Any previous experience with Medicaid day utilization issues related to disproportionate share payments,
and the processes that directly affect the calculation of DSH reimbursement
b. Any previous experience with the Medicaid Waiver programs
c. Any previous experience with Medicaid fraction days, SSI days, and dual eligible days
d. Any previous experience with Iowa Medicaid Enterprise or other State Medicaid Programs
e. Details on how the firm has successfully represented other state-owned academic medical centers in their
pursuits of similar services.
Attachments detailing the Supplier's proposal are limited to a maximum of 25 pages.
