NOTE: All removal and disposal of asbestos containing materials must comply with all existing Iowa Department of Natural Resources and U. S. Environmental Protection Agency guidelines and regulations.

Proposals must be addressed to: City Clerk, 105 E Third Street, Ottumwa, Iowa 52501 and plainly marked: Portion of Building on Terminal Ave – August 6, 2026. The request for proposal and contract conditions may be obtained from the Building and Code Enforcement Department, 105 E Third St, Ottumwa, IA 52501 or online at www.ottumwa.us .

Bid security deposit required. (See bid form). A 100% performance bond or irrevocable letter of credit stating the total cost of the project bid is required from the successful bidder on all removal projects.

DEMOLITION AND DISPOSAL OF A PORTION OF THE BUILDING ON TERMINAL AVENUE,
OTTUMWA, IOWA
Foundation and clean fill material from the site can be used as fill on the property. Fill dirt will be provided and

WORK REQUIRED
1. Demolish the portion of the structure (s) on the lot, level, and properly dispose of all structures located on the
property(s) at the address(s) indicated on the attached bid form to include exterior walls, interior walls, floors,
windows and doors, plumbing, electrical, heating and ventilation systems and their associated fixtures, ceilings, roof
supports and all roofing materials, foundations, etc. All demolition material is to be taken to the Ottumwa/Wapello
County landfill.
2. The selected contractor will not be required to pay any fee, except those mentioned herein, for the disposal of the
structure(s) listed in the attached bid form. All recyclable materials, appliances, tires, etc. shall be delivered to the
Ottumwa/Wapello County Recycling Center. Appliances are to be removed from the structure(s) PRIOR to the
demolition. Care is to be taken NOT to damage the appliances so as to release toxic waste to the atmosphere. Notify
the Recycling Center office at the time appliances or tires are dropped off at the Center. Failure to comply with these
instructions will result in a $100.00 deduct per appliance.
3. Kitchen and bath fixtures may be reclaimed or land filled. All salvageable material, brick, concrete, etc. is to be
separated and delivered to the Ottumwa/Wapello County Landfill. All recyclables and salvageable material not
separated and taken to the Ottumwa/Wapello County Recycling Center or to the Ottumwa/Wapello County Landfill
will be charged back to the contractor at twice the normal tipping fee rate for the entire load.
4. Remove foundation and basement walls, fill with clean fill and cover with 6" of humus soil to the contour of the
surrounding earth. Concrete may be broken up and disposed of at the landfill providing there is no exposed rebar.
Seed the filled/disturbed area with a standard grass seed mixture. Erosion control and silt fence or bollards may be
required on some properties. Clean entire lot of all brush, trash, garbage or other types of debris.
5. Contractors are responsible for all required legal notifications. Failure to follow the rules and requirements
associated with the Work Required Document, Bid Document, Notice of Letting Document or Contract will result in
the contractor paying for any and all damages.

B. SCOPE OF WORK
Methods and Procedures:
The property located 14802 Terminal St, Ottumwa, Iowa was inspected prior to scheduled
demolition activities which may disturb asbestos containing building materials in accordance
with NESHAPS (National Emission Standards for Hazardous Air Pollutants) regulations.
Field sampling methods were based on (NESHAPS) National Emission Standards for Hazardous
Air Pollutants) protocols. Representative samples of suspect building materials were taken
from Homogeneous Areas (HA-defined as similar in age, appearance and function). The
purpose of this inspection was to identify quantities and locations of asbestos containing
building materials prior to demolition of the structures. Bulk samples of suspect asbestos
containing materials (ACM) were analyzed by Polarized Light Microscopy (PLM) with dispersion
staining, as described in 40 CFR Part 763 and NESHAPs. Each sample was analyzed for the six
different types of fibrous asbestos forms, of which a percentage, by volume, is estimated and
summarized. If further analysis and quantification is warranted, this analysis is performed by
EPA 600/R-93/116 with 400 or 600 Point Count Procedure. Further analysis of samples may
also be performed at the client's request using Transmission Electron Microscopy (TEM).
Sample analysis was performed by Eurofins CEI. Eurofins is accredited by the National Institute
for Standards and Technology for Polarized Light Microscopy analysis under their NVLAP
(National Voluntary Lab Accreditation Program).
Asbestos Containing Building Materials (ACBM)s and their control during renovation or
demolition activities is regulated in Iowa by the Iowa Department of Natural Resources.

INSPECTION NOTES
In accordance with your request and authorization, Hawkeye Environmental LLC has performed
an asbestos survey for the project referenced above. It is our understanding that the subject
property will be demolished. Demolition has the potential to disturb all building materials. This
survey intends to determine if any of the materials with potential for disturbance are asbestos
containing.
The building is a vacant commercial building. The property is in poor condition for its age and
state of use.
Roof or roofing systems were sampled and included in this report.
A representative number of samples were collected from all suspect asbestos building
materials.
This property is located at the corner of Terminal Ave and Truman St at the Ottumwa Regional
Airport.
