The City of Show Low is extending an invitation for proposals for Aquatic Center Restroom

Remodel, City of Show Low Project FM-3424.

The project is located in the Aquatic Center building, 1100 W. Deuce of Clubs. There are

two small restrooms that will be converted into a single ADA accessible restroom. The

improvements include new plumbing fixtures, partitions, wallboard, paint, re-finishing concrete

floors, lighting, doors, paper and soap product dispensers, an adult changing station and other

necessary appurtenances. The scope will include necessary demolition and disposal of generated

waste materials. In-slab and in-wall plumbing will need to be reconfigured. This work must be

completed during the annual maintenance shutdown of the Aquatic Center from November

27 to December 17. Schematic plans are provided in the bid packet for the facility. These plans

were prepared for the purpose of illustrating general layout, materials, and dimensions and they

are available with the Bid Documents on the City website showlowaz.gov. The successful bidder

will be responsible to prepare permittable plans for the construction.

The proposals will be evaluated based on cost, value, and adherence to the preliminary drawings.

A schedule of values for the restroom will also be required as a part of the contractor’s proposal.

The contactor shall complete usable and operable facilities in accordance with all pertinent codes

and regulations as necessary to complete the job in a workmanlike manner.

Sealed Proposals will be received by the Engineering Department, City of Show Low, 180 N. 9th

Street, Show Low, AZ 85901 until Thursday, October 26 at 4:00 PM local time, The City of

Show Low reserves the right to select the proposal that best suits the City’s needs and may modify

the Scope of Work. The City of Show Low also reserves the right to reject any and all proposals

not deemed in the best interest of the City. A MANDATORY pre-bid conference will be held

on Thursday, October 12 at 2:00 pm, at City Hall, 180 N. 9th Street, Show Low, AZ.

Bid Documents are available on the City website; Showlowaz.gov. Written questions will be

I. Project Scope
The scope of project includes constructing upgrades for Americans with Disability
Act (ADA) compliance at a public restroom located on City of Show Low’s Aquatic
Center property. There are two small restrooms that will be converted into a single ADA
accessible restroom. The improvements include new plumbing fixtures, partitions, wall
board, paint, re-finishing concrete floors, lighting, doors, paper and soap product
dispensers, an adult changing station and other necessary appurtenances. The scope will
include necessary demolition and disposal of generated waste materials. In-slab and in-
wall plumbing will need to be reconfigured. This work must be completed during the

III. A. City Provided Facilities and Equipment
There are existing utilities (water and electrical power) at the project site
B. Contractor Provided Equipment
The Contractor will provide the following equipment and services for the
performance of this contract:
Labor, materials, equipment, incidentals, etc. for a complete and operable facility.
IV. Contractor Qualifications
The successful Proposer shall provide information regarding similar projects in
the past five years.
V. Pricing
A detailed schedule of values shall be provided with the Proposal. The costs shall
include all labor, equipment, materials, and incidentals required to provide a
complete project. If there are costs associated with the project that cannot be
determined or are excluded from the proposal, a detailed explanation shall be
provided along with an approximate cost. The contractor’s schedule of values shall
include a detailed schedule of values for major construction elements (for example:
concrete slab, foundation, masonry walls, framing, siding, roofing electrical,

plumbing, insulation, drywall, hardware and stalls, final cleaning and dumpsters,
equipment & tools, overhead and general conditions etc.). The schedule of values
will be used for calculating progress payments based on percentage complete for
each line item.
The building permit and utility connection fees will be waived by the City.
*NOTE: Bid prices shall include ALL applicable Taxes.
VI. Schedule
The proposal shall include a schedule showing the work to be completed in the
required time frame, November 27 to December 17. City staff anticipates the
project will be approved at the October 17 City Council Meeting. This should
allow lead time for ordering required materials.

VIII. Proposal Contents
Proposals submitted for this project shall address all requested information. Firms
are encouraged to keep the proposals brief and to the point, but sufficiently
detailed to allow for evaluation. Pricing is a part of this proposal; however the
selection will be made based upon price and other considerations which are
detailed in the section below. Proposals must include the following:
A. Cover letter (1 page maximum)
The cover letter shall include information regarding the type of business entity of
the proposer (LLC, Partnership, Individual, Association, etc.) entity, a statement
of the Proposers good standing, general company background, and
acknowledgement of any Addendums.
B. – Project Understanding and Experience
Identify any unique approaches or strengths that your firm may have related to this
project. Provide examples of similar projects your team has completed in the past
five years. Project contacts must be provided including a customer representative
name, phone number, and location. Describe how your team is uniquely qualified
to provide these services.

C. – Technical and Cost Proposal
This section shall include your work plan. It should include sketches and details
that will illustrate the scope. Include schedule information, project descriptions and
supporting graphics. A detailed cost proposal shall also be included.
D. - Signature
The proposal shall be signed by a representative authorized to bind the team and
shall expressly state the proposal is valid until the City Council awards the contract.
By signing the Letter and submitting a complete proposal, the Proposer gives the
following assurances and information and covenants that the Proposer is fully
qualified to provide the required services. Proposer further affirms that the
following submitted information is true and accurate and may be relied upon by the
in evaluating the proposal.
IX. Price Proposal
This document shall include the price for performing the work described in the
Project Scoping Section above. The City is expecting a turnkey project so any
listed exceptions should be minimal. If there are elements of the project whose
costs cannot be defined, an explanation should be included and a contingency for
the item shall be included in the cost. This information shall be included in Section
C of the proposal.
X. Submittal of Proposals

Three (3) copies of the proposals must be received by the due date for submittal:
