1. Overview
OVERVIEW AND INTENT
This RFP intends to solicit sealed proposals from qualified Contractor(s) to provide land
improvements in accordance with the construction plans and specifications.
The Contract Bid Documents include Base Bid project specifications and Additive Alternate project
specifications. The City will award the Contract to the Lowest Responsive and Responsible Bidder of
the Base Bid. Any or all of the Additive Alternate Bid Items may be included in the Construction
Contract prior to execution at the sole discretion of the City of DeBary.

SCOPE OF WORK OR SERVICES
Response to this Request for Proposal, RFP 16-26 is confirmation of the Contractor’s ability and the
resources necessary to utilize the Computer-Aided Design – CAD Drawings that are included with this
solicitation as the Contract Construction Documents, and the ability to do quantity take-offs and estimate
all plan quantities and to construct a fully functional complete project that is ready for use in accordance
with the data and specifications contained therein.
Prior to the Award of the Contract, the Contractor shall meet with the City of DeBary to review the Bid
Submission. For the meeting, the Contractor will be required to provide confirmation that the Contractor’s
quantity calculations accurately reflect the scope of the project and that the Contractor’s pricing is sufficient
to Construct the project as specified in the Request for Proposal Documents.
Prior to the start of Construction, the Contractor shall request the City of DeBary to issue a Notice Proceed.
Prior to issuance of Notice to Proceed, the Contractor shall submit to the City of DeBary for approval an
estimated schedule of progress payments based on the Contractor’s construction schedule for completion
of the Work for each lump sum Payment Item.

Preliminary
1. Mobilization.
2. Provision of Performance and Payment Bonds according to F.S. 255.05. (Page 11, #13)
3. Provision of Insurance according to all references within this RFP 16-26.
4. Indemnification
General Site Requirements
1. Site Surveying – provide construction layout of the project according to the Construction Plans,
Sheet C3.0 Site Demolition Plan, Sheet C4.0, Site Plan, Sheet 5.0, Paving Grading and Drainage
Plan, Sheet 5.1, Cross Sections, and Sheet 6.0 Utility Plan and Sheet L3.01, L3.1, L3.3, including
any relevant details provided. Site Surveying shall include as-built record drawings and
documentation produced by a Florida Licensed Professional Surveyor and Mapper, (PSM). The
Contractor shall submit as-built documentation to the City of DeBary in CAD and pdf format.
2. Erosion and Sedimentation Control and installation of silt fencing in accordance with Sheet C2.0
and Sheet C3.0.
3. Maintenance of Traffic shall be required for all Work by the Contractor that may affect traffic on
Ft. Florida Road. On-site Work shall not require Maintenance of Traffic.

4. Clearing and Grubbing – general clearing and grubbing according FDOT Standard Specifications.
Clearing and grubbing includes removal and disposal of existing barbed wire fence and removal
and disposal of existing livestock pens.
5. Dewatering shall include all labor, equipment, materials and fuel required for the all construction
operations for the entire project.
6. Tree Mitigation shall be required in accordance with the Construction Drawings Sheet L.50 and
Sheet l.51. Tree Mitigation includes removal of all trees as shown on Sheet L.50 and Sheet L.51.
7. Import fill – that Contractor shall provide all fill material as required to construct the project in
accordance with the Paving, Grading and Drainage Plan, Sheet C5.0, and any relevant details
provided.
8. Segmental Retaining Walls shall be provided by the Contractor. The structural design of the
retaining walls shall be included in the scope of this Pay item in addition to the provision of all
materials and labor required for the installation of the retaining walls. Submittal of the design and
construction drawings for the segmental retaining walls shall be required for City of DeBary
approval.
9. Soils Analysis and Density Testing – the Contractor shall provide a Certified Testing Laboratory
for soil sampling and analysis, density testing and reporting. Test reports shall be delivered to the
City of DeBary.
10. The Contractor is obligated to assure that its employees and agents notify the City immediately
upon encountering any endangered, threatened, or listed species such as the gopher tortoise,
scrub jay, or indigo snake. No such protected species or its nesting area shall be disturbed
without compliance with the procedures of the Florida Game and Fresh Water Fish Commission
and the U.S. Fish and Wildlife Service. The Contractor shall hold the City harmless from any
claims, damages, attorney’s fees, litigation expenses, costs and fines for the actions of the
contractor, its employees or agents in violation of applicable procedures.
Electrical
1. Utility Coordination – Florida Power & Light Company – provide coordination with FPL as required
to receive and install electrical conduits and splice boxes from Conduit and splice box materials
shall be provided by Florida Power and Light Company.
2. Provide all electrical equipment and installation of electrical equipment as shown on Sheet C7.3
for electrical power to the Sanitary Lift Station. Coordinate with FPL to energize the lift station.
Stormwater Drainage System
1. Site Grading – All site grading and includes Retention Pond and Retention Areas, Swales
Stormwater Pipe, Stormwater Structures, Mitered End Sections – as shown on Sheet C5.0, and
any relevant details provided.
2. Includes providing all filter cloth wrapped perforated pipe to be installed with the TrueGrid Pro
Commercial Paving System.

3. Provide all materials and labor to construct the complete Stormwater Drainage system as shown
on Sheet C5.0, and any relevant details provided.
Reclaimed Water System
1. Utility Coordination – Volusia County Water Resources and Utilities Department – includes all
inspections and testing and connections as required.
2. Provide and install irrigation Sleeves as shown on Sheet C6.0
3. Provide all materials and labor to construct the Reclaimed Water system as shown on Sheet
C6.0.
Potable Water System
1. Construction in compliance with the Construction Drawings, Sheet C1.1, General Notes, Potable
Water System, Note No. 1 thru Note No. 11.
2. Compliance with the Construction Drawings, Sheet C1.1, Potable Water Testing and Inspection -
Volusia County Water Resources and Utilities Department – includes all coordination of
inspections and testing and connections as required by Volusia County.
3. Provide all materials and labor to construct the complete Potable Water system as shown on
Sheet C6.0.
Sanitary System
1. Utility Coordination – Volusia County Water Resources and Utilities Department – includes all
inspections and testing and connections as required.
2. Private Sanitary Lift Station – Complete – according to Sheet C7.3
3. Provide all materials and labor to construct the complete Sanitary system as shown on Sheet
C6.0.
4. Provide 6’ height, vinyl screen fence around sanitary lift station according to Sheet L3.03.
Vehicle Access
1. TrueGrid Pro Commercial Paving System– the Contractor shall provide a complete paving
system according to limits and specifications shown on Sheet C5.0 and Sheet C7.1, and as
permitted through the St. Johns River Water Management District (Permit No. 214991 – 2). No
substitutions or variations from the specified TrueGrid System shall be allowed. The complete
paving system shall include; preparation of the subgrade, provision and placement of the
GeoTextile Fabric, provision and placement of #4 stone and the TrueGrid Pro Permeable Paving
System.
2. Six (6) Total Wheel Stops for five (5) handicap parking spaces and adjacent single (1) regular
space.
3. Pavement markings and signage for handicap parking spaces.
