The Alabama Department of Rehabilitation Services, VRS Lakeshore Office located at 201 London Parkway,
Suite 450, Birmingham, AL. 35211 is requesting the following items be added to our new Ford Explorers.

Line 1 to include the following:
3 each - Evaluator Hand Control Packages w/City Mode Item# FLQD-4PK ( -3.0 system 1 push rock on left and right and 1 push pull on left and right side with sure grip electric brake locks),
3 each - Left Foot Gas Pedal for evaluator package Item# FLPED-QD - 3.0 system,
3 each - Passenger side gas/brake for evaluator system - 3.0 system,
3 each - Sets Steering Orthotics Package - Spinner Knob, Straight Pin, Tri Pin, Palm Grip each with Clam Shells, and
3 each - Pedal Extension - Veigel Brake and Accelerator Petal Extension, Item# 54400k (Removable) and

Line 2 includes:  1 each - Labor (price submitted to include the total amount of labor needed (supplier may indicate on the line amount per hour or attach a quote).

NOTE:  Supplier must be within 100 miles of the location indicated above.
