The Purpose of this addendum is to provide responses to questions asked during the Inquiry

Phase of this Request for Proposal. Clarification was needed for a section of the RFQ. The contents of

this addendum are now officially a part of the Request for Proposal document. You must sign the

addendum acknowledgement and include all of this in your bid project.

1. On Page 2 of the RFQ, it is stated that we require “A completed Form A305CM”.

The OMES form A305 is now form D305.
