NOTICE OF REQUEST FOR PROPOSALS

FOR

TITLE XX SERVICES

(ADULT DAY CARE SERVICES, PREVENTION & INTERVENTION SERVICES, AND ADULT

PROTECTIVE CASE MANAGEMENT)

FOR THE PORTAGE COUNTY DEPARTMENT OF JOB & FAMILY SERVICES

SEALED PROPOSALS will be received by the Director of Internal Services, at the Portage

County Administration Building, 1st floor- room 114, 449 South Meridian St., Ravenna,

Ohio via the United States Postal Service or by UPS or FedEx courier only (no in person

delivery or drop-offs) until 2:00 p.m. (E.S.T.) on August 12, 2020 for the above.

Request for Proposal Forms and any information concerning the requests may be

obtained from JoAnn Townend, Director of Internal Services, Portage County

Administration Building, 449 South Meridian St., Ravenna, OH 44266. This

advertisement/notice for bids can also be found on Portage County’s website at:

www.co.portage.oh.us under County bid notices.

Successful proposer must comply with Equal Employment Requirements as specified in

Executive Orders 11246 and 11375.

The Commissioners reserve the right to reject any or all proposals or any part thereof.
