Description
It is the responsibility of the Applicant to reveiw all attachments listed in the section below, including the Request for Application Guidance. The issuance of the Application in no way constitutes a committment by IDPH to award a contract.

Excerpts from the Application:

1.01 Purpose

The purpose of this Application Guidance is to provide eligible applicants information on completion of the application for the State Youth Treatment Implementation (SYT-I) grant. Services covered by this application include efforts to expand and advance evidence-based treatment and recovery support services for substance use and/or co-occurring disorders among adolescents and transitional aged youth (TAY), ages 12 to 25, and their families/caregivers through SYT-I.

1.02 Project and Contract Period

Funding during the defined project period is dependent on approval of the Application, contractor performance during the fiscal year, compliance with general and special conditions of the contract, availability of project funds, program modifications, or any other grounds determined by the Department to be in the Departments best interest. The Department expects the contract period to be a one-year term from September 30, 2017 to September 29, 2018.

The issuance of this Application Guidance in no way constitutes a commitment by the Department to award a contract.

1.03 Eligible Applicants

Applicants must meet each of the following eligibility requirements for consideration.

Eligible Applicants are current SYT-I Contractors:

  • Heartland Family Service, Council Bluffs, IA

  • Prairie Ridge Integrated Behavioral Healthcare dba Prairie Ridge, Mason City, IA

  • Prelude Behavioral Services, Iowa City, IA

  • Youth and Shelter Services, Inc., Ames, IA

Electronic Communication Requirements

Applicant is required to maintain and provide to the Department, upon application, a current and valid email account for electronic communications with the Department.

Official email communication from the Department regarding this application will be issued from iowa.grants@webgrantsmail.com . Applicants are required to assure these communications are received and responded to accordingly

1.04 Service Delivery Area

Service Delivery area is the county represented by the eligible applicant.

Any service delivery area changes for this application process must be agreed to by existing contractors and applicable boards of health, and approved by the Department. Submit requests to Cara.Weis@idph.iowa.gov. Written approval must be obtained prior to submitting an application.

1.05 Funding

The source of funding is provided from a cooperative agreement from the Substance Abuse and Mental Health Services Administration, Center for Substance Abuse Treatment (SAMHSA/CSAT), SYT-I Project.

Applicants may apply for up to $130,000 for the total contract period for overall services, to include direct treatment and recovery support services, EBP implementation, and marketing. Actual total awards and individual contract funding levels may vary from that listed or funding may be withdrawn completely, depending on availability of funding or any other grounds determined by the Department to be in the Departments best interests.

As Year 3 is the final year of the project, additional funding will be awarded to subcontract and implement evidence-based practices in accordance with National Registry of Evidence-Based Programs and Practices (NREPP ).:

  • Heartland Family Service shall apply for up to $30,000 for the total contract period to subcontract with Multi-Dimensional Family Therapy (MDFT), International and the Comprehensive Adolescent Severity Inventory (CASI) developer. Purchase required training, certifications, training materials, and consulting services to increase the number of clinicians competent in MDFT. At minimum, purchase web-based CASI training statewide license for unlimited use through December, 31, 2018, web-based CASI data entry and clinical summary system for unlimited statewide use through December, 31, 2018, and assure statewide access to all CASI administration, training, quality assurance, and scoring materials.

  • Youth and Shelter Services, Inc. shall apply for up to $30,000 for the total contract period to subcontract with Multi-Dimensional Family Therapy (MDFT), International and the Comprehensive Adolescent Severity Inventory (CASI) developer.   • Prairie Ridge Integrated Behavioral Healthcare shall apply for up to $10,000 for the total contract period to continue to implement Motivational Enhancement Therapy/Cognitive Behavioral Therapy (MET/CBT). In consultation with the Department, provide a minimum of two (2) statewide Motivational Enhancement Therapy/Cognitive Behavioral Therapy (MET/CBT) trainings, provide a minimum of eight (8) certifications (paid by providers), and a minimum of four( 4) consultation calls (two (2) calls per training).
