General Information

A. Information for Applicants
The Commonwealth of Pennsylvania funds the Specialty Care Program (SCP), which provides
services and supports for Pennsylvania residents with beta thalassemia (Cooley’s Anemia), cystic
fibrosis, hemophilia, sickle cell disease, spina bifida, and neuromuscular and orthopedic conditions
(referred to as Child Rehabilitation; CR). The SCP goal is to assure individuals diagnosed with
neuromuscular and orthopedic conditions (clients) are able to lead as healthy and productive lives
as possible. The SCP is designed to enhance communication and service provision across health
care systems and providers, while providing clients equitable access to these enhanced services.
Through the tasks identified in Section B.4 Deliverables below, health care systems and providers
will be better equipped to act in partnership with clients to bring the full support of the clients’
families and community resources to improve care and begin to systemically remediate disparities,
alleviate barriers to care, and ultimately improve health outcomes and quality of life for all
Commonwealth residents with neuromuscular and orthopedic conditions.
The SCP seeks to build upon Pennsylvania’s integrated Medical Assistance delivery system within
each of the five HealthChoices regions as outlined in Table 1 – Region Names, Yearly Funding,
and Counties below. Integrated delivery systems assume responsibility for planning, providing,
purchasing, and coordinating all core services for the target population, as well as increasing
cooperation among health and social services organizations. Integrated systems reduce the cost
and increase the quality of care, improving health outcomes for people with chronic conditions.
The integrated delivery system seeks to be navigable and to utilize the input of community
stakeholders to increase access to care and support adherence to health care plans. The integrated
delivery system provides geographic coverage to maximize client access to the services they need
and reduces service duplication.
The SCP funds public health services that improve care coordination, increase access to care,
overcome health disparities, integrate physical and mental health services, and adopt a whole-
person approach to specialty care. Services must be client-centered, meaning that clients’ needs,
and preferences drive the individualized care planning and implementation. People with chronic
conditions are forced by the all-encompassing impact their diseases have on their lives to play an
active role in their own care management, but they do not always possess the necessary tools and
knowledge to do so effectively. Public health services close these gaps and assure that the client
has access to all necessary resources to attain their highest quality of life.
Through this RFA process, the Pennsylvania Department of Health (Department) is soliciting
applications for the Specialty Care – Child Rehabilitation Program (CR Program) from
Pennsylvania institutions and organizations. The Department is interested in funding applications
addressing public health services for Pennsylvania residents under the age of 22 years with
neuromuscular and orthopedic conditions. The overall goals of this funding are to promote access
