RFP #2022-48 - Community Health Needs Assessment- Community Health Improvement Plan

The United States Centers for Disease Control and Prevention (CDC) establishes the public health activities that all communities should undertake to protect and promote the health of all people in the "10 Essential Public Health Services (EPHS)." These activities begin with the process of assessment; in order to know how health can be improved, a jurisdiction must first understand its needs, strengths, and assets. Therefore, community health assessment and a community health improvement plan is a key function of health departments. The Allentown Health Bureau (AHB) is in need of services to: Conduct a community health assessment for the City of Allentown in collaboration with AHB staff. Provide project management, staffing and technical assistance to implement the community health needs assessment (CHNA) through to completion in a timely fashion. Gather and analyze data from the CHNA and other publicly available data sources to inform and create, in collaboration with AHB, a multi- year Community Health Improvement Plan (CHIP). Work with AHB and the City of Allentown to develop strategies to address the CHIP. Establish a framework for AHB and the City of Allentown to re-implement the CHNA and CHIP every 3-5 years.
