Unusual Occurrence Report (UOR) Tool

Introduction:

The Alaska Psychiatric Institute (API) is soliciting proposals for an electronic unusual event reporting tool. The tool will help reduce redundancies in API’s current system by bringing the reporting of medication errors, escalating patient behaviors, patient complaints/grievances into one reporting system.

Scope of Work:

The Department of Health and Social Services, Division of Alaska Psychiatric Institute, is soliciting proposals for an electronic unusual event reporting tool (standard healthcare risk management software). The Department wants assistance to reduce redundancies in our existing reporting system and to improve the timeliness of investigations of events. The consultant will develop a UOR tool that gathers reporting of medication errors, escalating patient behaviors, and patient complaints/grievances into one reporting system. All API staff are to have access to this reporting system.


Minimum Requirements:

The firm’s statement should address how they satisfy the following minimum requirements and experience necessary to provide these services outlined in this RFI.

  1. The offeror must have three (4) years in the last ten (10) of experience constructing UOR tools for Healthcare providers.
  2. Reporting system must have at least five (5) years of experience in the past ten (10) years utilized by other healthcare facilities. Link medical records to the reporting system. Each incident must be emailed to the incident preparer
  3. Reporting system must have at least four (4) years of experience seven (7) years providing history of healthcare facilities that utilized the service. Link medical records to the reporting system.
