The Department of Public Health & Environment (DDPHE), is seeking proposals from community partners that offer programs/services that align with the objectives of the Centers for Disease Control & Prevention (CDC) Public Health Infrastructure Grant.
BackgroundDDPHE was awarded a 5-year grant from the Centers for Disease Control and Prevention (CDC) for Strengthening U.S. Public Health Infrastructure, Workforce, and Data Systems (also known as the Public Health Infrastructure Grant (PHIG)). The intent of the PHIG funding is to: 1) improve public health outcomes by strengthening the capacity and systems related to the public health workforce and infrastructure in Denver communities, 2) strengthen and sustain community partnership development and engagement, and 3) direct resources in a way that supports diversity and health equity.
Supporting Community Partners who make up Denver’s Public Health Infrastructure Program FundingAs part of the Request for Proposal (RFP), DDPHE estimates the total awarded funding to be approximately $842,000 to community-based organizations that serve Denver communities. Organizations may submit one grant application requesting up to $75,000 for the contract term. The contract term is anticipated to begin in March/April 2024 and end November 30, 2024 with potential renewal funding through an annual renewal process each year from December 1, 2024 through November 30, 2027, contingent upon federal allocation and each awardees successful completion of year one of the project with compliance to the terms of the contract. Unobligated funds may be carried over into the next term per expanded authority granted by the CDC. Subrecipients will be notified no later than December 15th of each year of their carryover amount.

Organization must be (or be able to) register with the Colorado Secretary of State
Organization cannot be de-barred and must have (or be able to have) a certificate of non-debarment, active registration on SAM.gov, and federal UEI #
Organization must have at least 12 months of operation and hold an active nonprofit status 501(c)3 at the time of application. This includes those organizations with a fiscal sponsor incorporated as a 501(c)3 and registered in the State of Colorado.
Organization must be in the City and County of Denver and/or primarily serve Denver residents with awarded funds.
Organization must be a community-based organization working at the local level to provide services to communities that are economically and socially marginalized.
This grant opportunity is 100% funded by the Centers for Disease Control and Prevention of the U.S. Department of Health and Human Services (HHS).
