Public Health Infrastructure Grant - Supporting Community Partners

• Organization must be (or be able to) register with the Colorado Secretary of State

• Organization cannot be de-barred and must have (or be able to have) a certificate of non-debarment, active registration on SAM.gov, and federal UEI #

• Organization must have at least 12 months of operation and hold an active nonprofit status 501(c)3 at the time of application. This includes those organizations with a fiscal sponsor incorporated as a 501(c)3 and registered in the State of Colorado.

• Organization must be in the City and County of Denver and/or primarily serve Denver residents with awarded funds.

• Organization must be a community-based organization working at the local level to provide services to communities that are economically and socially marginalized.

The City strongly encourages applications from nonprofits that provide services to communities that have been economically or socially marginalized, are composed of people from racial and ethnic groups, are medically underserved, and those disproportionately affected by COVID-19 or other priority public health problems such as:

• Communities of color (Asian American, Black, Indigenous, Latino, etc.)
• Older adults
• People experiencing poverty or homelessness
• Immigrants and refugees
• People living with disabilities
• Lesbian, Gay, Bisexual, Transgender, Queer (LGBTQ)
• Veterans
• Communities disproportionately impacted by COVID-19

Sample activities to consider include:

• Improve collaborative systems and processes with behavioral health partners or provide behavioral health services in underserved communities.
• Engage with communities to identify, prioritize, and address key health needs and gaps through collaborative processes.
• Engage with partners in the health care system to improve health service availability and access to care.
• Develop and implement multisector or system strategies to increase access to social services.
• Build relationships with educational programs that promote the development of future public health workers, including partnerships to create pathways into the public health workforce.
• Connect communities to services that support the whole person by providing robust linkages and navigation for community resources.

Required activities include:

• Identify community health needs, inform public health intervention programs, and assist with public health workforce recruitment strategies for public health across Denver by:
  • Participating in DDPHE’s Community Health Improvement Plan and Community Health Assessment.
  • Participating in DDPHE’s Health Equity Community Advisory Group (CAG).
• Develop a language access plan and protocol to communicate with and provide services to individuals with limited English proficiency. In compliance with Title VI of the Civil Rights Act of 1964, 2 C.S. § 561 et seq. (Act 172 of 2006), and City and County of Denver’s Executive Order NO. 150: Citywide Language Access Program
• Participate in the evaluation of their project with the technical assistance support of a DDPHE evaluator.
