1 INTRODUCTION
The City of Hot Springs, Arkansas, is seeking proposals from certified Contractor for pest control services as a Term
Contract (TC). This Request for Proposals (RFP) contains all the information necessary to prepare and submit a
proposal. Prospective Respondents are advised to study this RFP completely and follow the instructions exactly.

3 SCOPE OF SERVICE
The following scope of service describes the desired requirements for this service. The Contractor shall be licensed
to engage in pest control service work. The Contractor shall furnish all labor, supervision, transportation, tools,
materials, and supplies necessary to perform pest control services in accordance with the guidelines established by
Federal, State, and Local Government laws, rules, and regulations. Inspecting and controlling all vermin, such as,
but not restricted to: fleas, stinkbugs, ants, wasps, bees, mice, roaches, etc., at the locations. Preventive and
control treatment for all types of termites (ground Formosa, above ground, dry wood, carpenter ants, powder post
beetles, and wood decay fungi). Complete the "Bid Line items" on Line Item tab.
The Contractor shall provide service to the locations in the frequency shown and as necessary to control pests.
Service to be performed by the Contractor that is required more frequently than is shown in the RFP will be at no
additional cost to the City. In other words, the Contractor is required to provide service as often as needed to
adequately control pests for the amount of their proposal.
The Contractor shall obtain the signature of a City employee, preferably a supervisor or office manager when
service is performed at a City location. Monthly invoices are to be submitted to the City. Contractor's employees who
perform service at City locations are to wear or display identification that they represent the Contractor.

For services at the Training Center, the awarded Contractor is required to contact the Police Department before
arriving at the locations.

The City's preference would be to award the RFP to one (1) Contractor for all location, but reserves the right to
award by location. The contract resulting from this RFP will be for one (1) year with five (5) twelve (12) month terms,
subject to annual budget approval by the Hot Springs Board of Directors. Contract will automatically renew annually
unless either party supplies thirty (30) day written Notice of Cancellation. Any escalation in price shall be subject to
paragraph 16 of the Terms and Conditions - RFP. City reserves the right to accept or decline any and all price
increases.
4 STATEMENT OF REQUIREMENTS
The following requirements from the Respondent will be shown below on "Certificates Required" and "Additional
Requirements."
5 Certificate Required
Certificates for coverages and items below shall be submitted by the Respondent with the proposal. If nothing listed
below, no insurance requirements apply.
6 General Liability (Minimum Amount) $500,000
Respondent is required to submit the certificate in the response attachments.
7 Business License
Respondent shall provide a copy of your company's/firm's business license certificate. The successful Respondent
may be required to obtain a City of Hot Springs business license. For business services within the City of Hot
Springs, the City will honor business licenses from cities within the State of Arkansas.
8 Certificate of Good Standing with the State's Secretary of State
Provide a copy of the Certificate of Good Standing with the Secretary of State's.
9 Additional Requirements
The Respondent are hereby notified that the following additional items apply to this procurement as noted.
1 This procurement is subject to an escalation clause
0 Any price increase shall be presented to the City in writing and follow guidelines established under "Escalation
Clause" in the Terms and Conditions.
1 This procurement is not subject to federally required certifications/contract clauses.

1 PROPOSAL REQUIREMENTS/EVALUATION
2 Respondents shall submit proposals that include the categories enumerated hereinafter. Proposals will be
evaluated in accordance with the City's evaluation criteria based solely upon the proposal contents; therefore,

1 A. QUALIFICATIONS/EXPERIENCE/REFERENCES
3 Respondents shall describe and provide detail of the firm’s qualifications and experience to perform this work
(including expertise, training, certifications, licenses, etc.). Include a list of references with the company name,
contact information, and address.
1 B. PROPOSED METHOD OF SERVICE
4 Respondents shall detail the approach they will take in achieving the scope of services.
1 C. CAPACITY AND AVAILABILITY
5 Describe the organization, main areas of business, equipment availability, and resources for performing service to
the City.
