Sealed bids for Lining of 3 Culverts on County Road 85 - Ashford 22, C8524569 and C8524439 , according to specifications, will be received by the undersigned, at the Department of Public Works Facility, 8810 Route 242, Little Valley, New York , until Friday, July 20, 2018 at 1:45 P.M. after which they will be publicly opened at 2:00 P.M. (at the same location), by the undersigned, under the direction of the Public Works Committee of the Cattaraugus County Legislature. Each bid, at the time it is received, will be stamped showing the date and time of receipt. All bids must be sealed and clearly marked as follows: (Any bid not clearly marked will not be considered.)

The project will consist of relining 3 existing culverts with an in-place cured system.

Copies of the proposed Contract Documents, Plans, Specifications and Instructions to Bidders will be available Monday, June 25, 2018 , and may be obtained online at www.cattco.org/bid-request or secured at Cattaraugus County Department of Public Works, 8810 Route 242, Little Valley, New York 14755 . Phone Dawn Smith, @ (716) 938-9121, ext. 2465 . There will be a $50.00 charge for each set of specifications, plus $8.00 postage if mailed. Checks are to be made payable to the Cattaraugus County Treasurer . The specifications for this project will be available for examination at the office of Greenman-Pedersen, Inc.; 403 Main Street, Suite 330, Buffalo, NY 14203 and Southern Tier Builders Association; 65 West Main St., Falconer, NY 14733 , & on their web site. Cattaraugus County Local Law 12-2012 as ammended by Local Law 5-2015 requires that the County provide a copy of the Cattaraugus County Vendor Responsibility Form to the Low bidder. The low bidder will have 5 business days to return the completed form unless the form is mailed by the county to the vendor, in which case they will have 10 business days from the date of the mailing to return the form. Failure by the low bidder to submit the form within the above time frame will lead to the automatic rejection of their bid. The contractor must also ensure that all subcontractors to be used on the project complete this form and submit it to the County for approval within 5 business days of the preconstruction meeting. Failure to do so may lead to the rejection of the subcontractor at the County's discretion.

Currently the Commissioner of Public Works has approved the CCA-2 (New York State Vendor Responsibility Questionnaire For-Profit Construction) with attachments A, B, and C to serve as the Cattaraugus County Vendor Responsiblity Form.

All bids must be sent or delivered to Cattaraugus County Department of Public Works, 8810 Route 242, Little Valley, NY 14755. Attention Dawn Smith.

The full deposit, according to General Municipal Law, Less any postage costs, will be refunded for complete sets with no missing pages, returned in good condition (NOT MARKED IN OR WRITTEN IN) within 30 days of the award of contract. No refunds will be made for sets returned later than 30 days after the award of contract.

All bids must be accompanied by a NON-COLLUSIVE BIDDING CERTIFICATE ad Bid Bond in the amount of 5% of bid total. All substitutions are to be submitted at time of bid as per the contract documents. No substitutions will be considered after the bid date. Any bid submitted without such certification and bid bond will not be accepted. Any bid not meeting ALL specifications will not be considered.

The County Legislature reserves the right to reject any or all bids, to waive any informalities, and to accept the lowest responsible bid.

LINING OF 3 CULVERTS ON COUNTY ROAD 85
TOWN OF ASHFORD
CATTARAUGUS COUNTY
Description: This project is located in the Town of Ashford, Cattaraugus County, New York. The
target work area is County Road 85, between the intersection of Edies/Dutch Hill Roads and the
intersection with County Road 86. The roadway length is approximately 0.75 mile long. The work
will consist of lining 3 corrugated metal culvert pipes with cured in place liners. Incidental work will
include clearing and grubbing at the end of the culvert in order to provide access, and work zone
traffic control.

Date of Completion: Within time period stipulated in NYSDEC and/or Army Corps of Engineers
permit(s), but no later than December 31, 2018.
Work to be done:
The Contractor shall furnish all materials, equipment, tools and labor of every
kind required to line the existing culvets, and all other incidental work in the
most substantial and workmanlike manner, and do everything required by the
Contract Documents as defined herein.
The contract drawings give specific limits and dimensions of work to be
completed. A general scope of work is given on page E-8 that denotes work
to be done by the Contractor. All items of work are to be included in the unit
prices bid listed in section A. Survey and stakeout is the responsibility of the
Contractor and shall be included in the itemized bid for item 625.01.
