I. STATEMENT OF WORK
A. INTENT
It is the intent of these specifications, terms, and conditions to describe the services
requested by the Alameda County Health Care Services Agency, Public Health
Department, California Children’s Services (hereafter, County). County is seeking an
experienced contractor to provide family-focused care management and resource and
referral navigation services (“Family-Centered Care Services”) to the families of clients
enrolled in Alameda County’s California Children’s Services Program and are not
members of a Medi-Cal Managed Care Plan.
County intends to award a nine (9) month contract (with the option to renew annually
for a maximum of five [5] years) to the Bidder whose response conforms to the IRFP and
meets the County’s requirements. An initial amount not to exceed $72,000 may be
available during the initial contract term. Future amounts may be negotiated in
subsequent years upon renewal to cover the services outlined in this IRFP, dependent
on availability of future contract funding.
B. SCOPE
Alameda County’s California Children’s Services program seeks to contract direct client
services including family navigation, care management, youth to adult transition
planning, and service linkages as well as training a training module for Public Health’s
CCS Enhanced Care Management (ECM) staff. The selected bidder shall perform a
needs assessment for clients in the CCS program who are not members of a MediCal
Managed Care Plan to determine which of these components will benefit clients and
their families.
Core Components
Awarded bidder shall be prepared to provide the following core components for this
project:
Family-Centered Support: recognizes the role families need to have in the
decision making and care for their children. Family members are experts on
their child’s abilities and needs. All families are unique, and their needs are
influenced by their culture, history, and exposure to trauma.
Family Navigation: helps families with accessing community resources,
educating families to advocate for themselves, and participate in the care team.
Care Management: ensures coordination of a client’s specialty medical care, and
referrals to services to meet psycho-social needs.
Youth to Adult Transition Planning: supports a youth’s transition to adult care
and services.

Service Linkages: connections between health care providers, government
programs, and community organizations to support the family and client’s well-
being.
Training Module: provide a 6- week training module for Public Health’s CCS ECM
providers.
Population of Focus
Children and youth ages zero (0) to twenty-one (21) with special health needs enrolled
in Alameda County’s California Children’s Services (CCS) program and who are not
enrolled in a MediCal Managed Care Plan.
C. BACKGROUND
Family Health Services (FHS)
The mission of the Family Health Services (FHS) Division of the Alameda County Public
Health Department is to improve the health and well-being of our diverse families with
compassionate, comprehensive, and collaborative services. The FHS division provides a
range of services including educational, preventative, diagnostic, and treatment services
for, children, youth up to age 21, and parents. FHS programs aim to ensure access to
care, reduce infant and maternal mortality and improve the health and wellbeing of
mothers, fathers, families with young children, and children/youth with special health
care needs. Programs operated in the division include: California Children’s Services
(CCS); Child Health and Disability Prevention (CHDP); Health Care Program for Children
in Foster Care (HCPCFC) Developmental Disabilities Council (DDC); and the Maternal,
Paternal, Child, and Adolescent Health Program (MPCAH).
California Children’s Services (CCS)
California Children’s Services (CCS) works with hospitals and community agencies to
provide children with complex medical needs and their families with the right care, at
the right time, at the right place. CCS is the oldest and largest public health program in
the state of California. CCS provides medical case management services for children and
youth with special health care needs (CYSHN) under 21 years who have complex,
chronic, often disabling and/or life-threatening medical conditions that require specialty
medical care. Most CCS families have low incomes and are Medi-Cal beneficiaries. The
CCS Medical Therapy Program provides occupational and physical therapy to children
and youth with medically eligible conditions regardless of income, mostly at school-
based sites throughout the County.
The selected Bidder will work with both the CCS case management and medical therapy
programs to provide trauma-informed, linguistically appropriate, and culturally
responsive services. Services under this IRFP will help Alameda County CCS meet
performance measures set by the State for family participation and planning for
transition from pediatric to adult services.
