PART ONE: EXECUTIVE SUMMARY
PURPOSE. This is a Request for Competitive Sealed Proposals (RFP) under Section 125.071 of the Ohio Revised Code
(ORC) and Section 123:5-1-08 of the Ohio Administrative Code (OAC). The Department of Administrative Services (DAS),
Office of Procurement Services, on behalf of the Ohio Department of Health (the Agency), is soliciting competitive sealed
proposals (Proposals) for Evaluation Services for Tobacco Use Prevention and Cessation Programs and this RFP is the result
of that request. If a suitable offer is made in response to this RFP, the State of Ohio (State), through DAS, may enter into a
contract (the Contract) to have the selected Offeror (the Contractor) perform all or part of the Project (the Work). This RFP
provides details on what is required to submit a Proposal for the Work, how the State will evaluate the Proposals, and what will
be required of the Contractor in performing the Work.
This RFP also gives the estimated dates for the various events in the submission process, selection process, and performance
of the Work. While these dates are subject to change, prospective Offerors must be prepared to meet them as they currently
stand.
Once awarded, the term of the Contract will be from the award date through 06/30/21. The State may solely renew this
Contract at the discretion of DAS for a period of one month. Any further renewals will be by mutual agreement between the
Contractor and DAS for any number of times and for any period of time. The cumulative time of all mutual renewals may not
exceed four (4) years and are subject to and contingent upon the discretionary decision of the Ohio General Assembly to
appropriate funds for this Contract in each new biennium. DAS may renew all or part of this Contract subject to the
satisfactory performance of the Contractor and the needs of the Agency.
Any failure to meet a deadline in the submission or evaluation phases and any objection to the dates for performance of the
Project may result in DAS refusing to consider the Proposal of the Offeror.
BACKGROUND. Tobacco use is the single most preventable cause of death and disease in the United States today. Tobacco
use increases the risk for lung and other cancers and for cardiovascular and respiratory diseases. The American Cancer
Society (ACS) estimates that cigarette smoking is responsible for one of every five deaths in the United States, or more than
430,000 deaths per year.
The mission of the Tobacco Use Prevention and Cessation Program (TUPCP) at the Ohio Department of Health (ODH) is to
reduce the prevalence of tobacco use in Ohio and the activities of the program are focused in three different areas:
A. Preventing the initiation of the use of tobacco, with an emphasis on youth prevention,
B. Increasing the number of Ohio tobacco users who quit using tobacco, and
C. Decreasing the exposure of Ohioans to secondhand smoke.
The Offeror will assist in the development and implementation of a strategic evaluation plan to answer prioritized evaluation
questions and will assist the program in using evaluation data to make necessary changes to programming to maximize
efficiency and effectiveness of programming and services to decrease the burden of tobacco on Ohio.
OBJECTIVES. DAS has the following objectives that it wants this Work to fulfill, and it will be the Contractor’s obligation to
ensure that the personnel the Contractor provides are qualified to perform their portions of the Work.
The Ohio Department of Health (ODH) on behalf of the Tobacco-Use Prevention and Cessation Program (TUPCP) is seeking
proposals from qualified firms and organizations to conduct evaluation services for TUPCP consistent with development and
implementation of a strategic evaluation plan. This includes evaluation of the programs and promotional efforts funded by the
TUPCP, including the Ohio Tobacco Quit Line (OTQL). The Offeror may also provide evaluation training and technical
assistance and assist in oversight of ODH’s community-based tobacco-use prevention and cessation grantees. The Offeror
will collect and analyze data collected by programs and statewide surveys; and prepare reports and project summary reports
based on evaluation findings and customized reports as needed. The evaluation methodologies may include formative,
process and outcome.
The primary deliverables related to TUPCP for any Contract resulting from this RFP will be:
A. Regular communication with the ODH contract manager overseeing the project.
B. Facilitation of a Strategic Evaluation Planning Team.
C. Development, maintenance, and implementation of the strategic evaluation plan for the TUPCP.
D. Participation in TUPCP program staff meetings at least monthly and facilitation of 2-4 meetings of the Evaluation
Planning Team, annually.
E. Ongoing evaluation of the Ohio Tobacco Quit Line (OTQL) with a mid-year and annual report.
F. Completion of at least seven (7) formal individual evaluations of program activities (at least two for the OTQL).
G. Assist in the development, implementation and analysis of grantee performance measures.
H. Participate in discussions and opportunities to share information with federal and state partners on Ohio evaluation
activities.

I. Provide ongoing technical assistance and smaller evaluation projects, as required.
The Contractor shall furnish its own support staff as necessary for the satisfactory performance of the Work described above.
Unless otherwise specified in the contract, ODH will not provide any staff, services, or material to the Contractor for the
purpose of assisting the Contractor in the performance of the contract.
CALENDAR OF EVENTS. The schedule for the Project is given below and is subject to change. DAS may change this
schedule at any time. If DAS changes the schedule before the Proposal due date, it will do so through an announcement on
the State Procurement Web site area for this RFP. The Web site announcement will be followed by an addendum to this RFP,
also available through the State Procurement Web site. After the Proposal due date and before the award of the Contract,
DAS will make scheduled changes through the RFP addendum process. DAS will make changes in the Project schedule after
the Contract award through the change order provisions located in the general terms and conditions of the Contract. It is each
prospective Offeror’s responsibility to check the Web site question and answer area for this RFP for current information
regarding this RFP and its calendar of events through award of the Contract. No contact shall be made with agency/program
staff until contract award is announced.

During the performance of the Work, a State representative (the “Agency Project Representative”) will represent the Agency
and be the primary contact for matters relating to the Work. The Agency Project Representative will be designated in writing
after the Contract award.
