Description:

The purpose of this Request for Proposals (RFP) is to solicit offers from qualified wellness vendors to provide health screening, health risk assessment, and health coaching to Forsyth County in support of the County’s wellness program. The on-site clinic services help support a continuum of care and support the wellness program. The vendor will work in collaboration with other vendors who provide Chaplaincy and EAP service provider. This collaboration will support a three-prong approach to employee well-being.
