REQUEST FOR APPLICATIONS
1. Request for Services
Purpose and Overview
This Request for Applications (RFA) is published to solicit applications for a high
level clinical audiologist who has experience with complex pediatric audiologist
diagnostics with expertise in quality assurance oversight to provide consultation
on activities related to the Department of Health and Human Services’
(Department) Early Hearing Detection and Intervention Program, pediatric
audiology diagnostic centers and hospital newborn hearing screening programs.
The Department anticipates awarding one (1) contract for the services in this
RFA.
Scope of Services
The selected Applicant must provide clinical audiological expertise to
provide oversight and consultation for quality assurance activities
relative to infant hearing screenings.
The selected Applicant must provide training to newborn screening
program staff, statewide, to ensure that children at risk for hearing loss,
and ultimately subsequent speech and learning delays, can be
effectively identified and referred for diagnostic evaluation; early
treatment; and remedial services.
The selected Applicant must provide audiological consultant technical
information to the Department’s Early Hearing Detection and
Intervention (EHDI) Program staff related to screening diagnostics;
early intervention; follow-up; and any other aspect of the program
structure and management as needed.
The selected Applicant must serve as EHDI liaison to the audiology
professional community, as needed or requested by each diagnostic
center.
The selected Applicant must provide technical and clinical consultation
to community audiologists and service providers related to the follow-
up of identified infants and young children, as needed.
The selected Applicant must provide education to healthcare
professionals, as needed or requested, relative to the current Joint
Commission on Infant Hearing (JCIH) position statement.
The selected Applicant must participate in the meetings listed below:
EHDI monthly team meetings (12).
EHDI Quality Improvement meetings (3).
The selected Applicant must attend the annual National EHDI
Conference.

The selected Applicant must provide pediatric audiology consultation
upon request from the pediatric diagnostic center.
The selected Applicant must participate in compiling and reporting data
to the Department for the Centers for Disease Control and Prevention
(CDC) Hearing Screening Follow-Up Survey report.
The selected Applicant must provide information and/or resources for
diagnostic centers; NH audiologists; and/or the medical community
related to NH EHDI protocols.
The selected Applicant must provide training to birth hospitals and birth
centers, as needed, based on annual hospital performance measures
or upon request of the EHDI Coordinator in the event of staff changes
that result in new screeners and/or equipment changes.
The selected Applicant must provide education to early supports and
services personnel statewide as requested to improve follow-up
process for infants and young children diagnosed with hearing loss.
The Department seeks to actively and regularly collaborate with
providers to enhance contract management, improve results, and
adjust program delivery and policy based on successful outcomes.
Performance Measures/Deliverables
The selected Applicant must participate in all of the
scheduled EHDI program team meetings and Quality
Improvement meetings.
The selected Applicant must respond to any direct requests
for NH EHDI audiology consultation within five (5) business
days.
The selected Applicant must respond to any request for
hearing screening training within five (5) business days.
The selected Applicant must respond within five (5)
business days to any request for education to early supports
and service personnel to improve the follow-up process for
infants and young children diagnosed with hearing loss.
Data and Reporting Requirements:
The selected Applicant must submit annual performance
measure outcome reports to the Department.
The selected Applicant must submit, on an annual basis, or
as requested by the Department, data on performance
measure outcomes to identify and reflect data trends.
The selected Applicant must develop and submit a
corrective action plan to the Department for any
