Community Paramedic East Central Health District

The Georgia Department of Public Health, (DPH) Epidemiology Section conducts surveillance for the occurrence of diseases and other health conditions that effect Georgians. As part of a project for DPH, this solicitation is being conducted to solicit bids for issuance of a single contract for a Community Paramedic (CP) to work collaboratively to provide follow-up care service needs for high-risk stroke patients in the East Central Georgia Health District (specifically Columbia, Jefferson, and Richmond Counties) service areas. In cooperation with Augusta University Medical Center, the Supplier¿s CP shall implement and evaluate an innovative Community Paramedicine Program. This Program shall utilize the Supplier¿s CP to provide follow-up care services to high-risk stroke patients with hypertension who have been discharged home or with home health services from Augusta University Medical Center.
