LEGAL NOTICE

Sealed proposals for Community Based Services, and Mini-grants to be provided to Senior Citizens (individuals 60 years or older in need of assistance to remain independent) residing in Trumbull County, Ohio, for 2024-2026 funding will be received by the Board of County Commissioners of Trumbull County, Ohio, at the office of the Board in the County Administration Building, 160 High Street, N.W., Warren, Ohio 44481, until 11:00 a.m. Warren, Ohio Time, on the 31st day of May, 2024 at which time they will be publicly opened and read. Organizations interested in applying for funding may pick up a Proposal/Application Packet in the office of the Board of Commissioners. A request may also be made via e-mail at :SLJURKOV@co.trumbull.oh.us. The email request must include the full name and address of the organization requesting funding, contact information including name, phone number, fax number and email address. Complete the email subject line with “Senior Community Services RFP”. Electronic documents will be either Word Format or PDF.

The Board of Trumbull County Commissioners is requesting applications from Service Provider organizations to provide Adult Protective Services (guardianships, protective services/emergency intervention), In-Home Services (home delivered meals, adult daycare, chores, and homemaker/personal care), Other Programs-Specific Funding, and Mini-grants. The service contracts will be for a 24-month period beginning as early as September 1, 2024, and ending August 31, 2026, except any current provider’s contracts with a contract ending date after August 31, 2024, whereas, if awarded for 2024-2026, the twenty-four-month contract will begin upon the completion of the current immediate contract.

Bids must be made in the general format using designated forms prescribed by the County and shall be filed in a sealed envelope at the time and place hereinbefore designated marked 2024-2026 Senior Levy Services RFP, full name, and address of bidding provider, and addressed to the Clerk of the Board of Trumbull County Commissioners.

Attention is directed to the requirement that each proposal must be accompanied by a Non-Collusion Affidavit duly signed by the bidder, and a Delinquent Tax Affidavit duly signed by the bidder—required by Section 5719.042 of the Ohio Revised Code—thereof. No payment shall be made on any Contract for which no such affidavit has been submitted.

Attention is also directed to all bidders that Trumbull County adopted a “New” Drug and Alcohol Free Workplace Policy on September 07, 2016, and the terms of this Policy also apply to any contractors directly reimbursed for services through the County; a copy of the Policy to be part of proposal, and that each proposal be accompanied by a “Sign-Off Sheet” duly signed by the bidder thereof.

Attention is also directed to all bidders that this notice has been posted on the County’s Internet Site on the worldwide web and found at http://www.demandstar.com/. (This notice may also be found on the Commissioners website at www.commissioners.co.trumbull.oh.us.)

The Board of Trumbull County Commissioners reserves the right to reject any or all bids submitted and to waive any informalities or irregularities in any bid received, and to determine the lowest and best responsive, responsible bidder(s), in accordance with the methods and criteria in the bidding documents.
