RFP #HSA015 - RFP-HSA015 Preventative Wraparound Services

The San Joaquin County Human Services Agency (HSA) is soliciting proposals for Preventative Wraparound Services. Preventative Wraparound is a family centered, strength-based, needs driven planning process for creating individualized services. The Preventative Wraparound Services priority population consists of families with children ages 0-5 years old, specifically Black and Latino children, families with children ages 11-15 years old, who are least likely to reunify after entering foster care, and pregnant and parenting foster youth. It supports children, youth, and their families, facilitating access to normalized and inclusive community options, activities, and opportunities.
