IFB On-call pickup, removal, clean up, and disposal of Hazardous Waste within District 08

Contractor shall furnish all labor, equipment, supplies and materials necessary to provide On-call
pickup, removal, clean up, and disposal of Hazardous Waste within District 08 to Caltrans.
Bidder shall possess at the time of bid submittal and for the duration of the Agreement, a valid and
current Motor Carrier Permit issued from the California Department of Motor Vehicles (DMV).
Caltrans will verify bidder¿s Motor Carrier Permit, it is not necessary to provide a copy.
Bidder must have at time of bid submittal and for the duration of the contract, a valid, current
Class A General Engineering Contractor license and a Hazardous Substance Removal (HAZ) Certification license issued by the CSLB, for the type of work to be performed.   Hazardous Materials Certificate of Registration issued by U.S. Department of Transportation Pipeline and Hazardous Materials Safety Administration.
Hazardous Materials Transportation License issued by California Highway Patrol (CHP).
Waste Tire Hauler Registration Decal for each vehicle with effective and expiration dates issued by Cal Recycle.
Hazardous Waste Transporter Registration issued by the California Department of Toxic Substances Control.
Transporter EPA ID (Section 3010 of the Resource Conservation and Recovery Act)
Bidder(s) shall possess at the start of work and for the duration of the Agreement, a valid and
current: Hazardous Waste Operations and Emergency Response HAZWOPER Certificates,
both for the initial 40 hours of training and most current eight (8) hour refresher training.
