Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice and is current as of FAC 2005-24, DCN 20070913, and AFAC 2008-0128. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The North American Industry Classification System code (NAICS) is 721110, the size standard of which is $5,000,000.00.

DESCRIPTION:     CLIN 0001 Conference Facilities & Rooms, QTY 1, U/I LUMP SUM (Include any and ALL fees under CLIN 0001.     Do not assume the attendees will pay additional fees not included in your offer.), and CLIN 0002 Evening Receptions QTY 2, U/I EACH. (Note:     Due to fiscal rules, CLIN 0002 will not be funded under this contract, but paid for by the conference attendees in the form of a Conference Fee.)

Name of Event:     Connected Hospital of the Future Symposium and Combatant Command Surgeons Conference.     The purpose of the Connected Hospital of the Future Symposium and Combatant Command Surgeon?s Conference is to educate and familiarize the Department of Defense Medical Healthcare System Senior Leadership on the resources available to facilitate the connected hospital of the future.    

CLIN 0001 Conference Facilities and Rooms
MINIMUM Requirements:
The meeting room and lodging rooms must be available April 20 through April 24, 2008,
Requires executive lodging accommodations including private conference calling and space for meetings that can seat 3-5 executives comfortably,
Lodging accommodations must provide a suite that has a sleeping area separated from the sitting area.     The suite must also contain a wireless and high speed internet access, mini-bar, and entertainment unit (i.e. TV, radio, etc.),
Require a 4 Diamond rated accommodation in accordance with AAA rating system, which indicates the extensiveness of services, amenities, and decor; including executive suites at a minimum size of 375 sq ft,
Must be within 6 miles of the Miami international airport,
Must participate in Federal Tax Exempt Program,
Must have an integrated sound system in the meeting room.

Number of Rooms Requested:     30 Executive level Suites described above.

Break Snacks:
Must provide the following snacks during the morning of April 21-24:
Coffee, Hot tea, Water, 2 Dozen Croissants, 1 Dozen assorted muffins, 1 Dozen assorted Danish pastries, 20 servings of fresh sliced seasonal fruit with berries served on platters, and must provide condiments (i.e. butter, jam, jelly, sugar, creamer, etc.)
                                    
Must provide snack during the afternoon of April 21-24 with the following:
Coffee, Hot tea, Water, Mineral water, Assorted sodas, and assorted cookies served on platters.

Audio and Visual Requirements:
1 Projection Screen 6x8 feet with black dress kit (skirting around the screen),
1 Speaker podium with built in speaker microphone,
1 Wired sound system in the meeting room,
2 video and audio connection cables for two laptops in the meeting room,
1 Work stand in the back of the room to house the laptops (with dress kit).

Meeting Room:
1 Meeting room each day for 4 days (April 21-24),
Meeting room minimum size of 734 Sq Ft,
Must have a minimum seating capacity of 40 people,
Must have 1 U-Shaped configuration that can accommodate 25 seats on the outer ring of the U-Shape,
15 Seats around the back walls of meeting room,
Must provide a minimum of two internet connections in the meeting room (if wired, one connection must be near the workstation).

CLIN 0002 Evening Receptions: Must provide a private casual area for evening reception/meet and greet within the lodging area which will accommodate an evening reception on Monday, April 21 and Wednesday, April 23, 5:30 pm to 7:30 pm.:

PLEASE NOTE:
Quotes will be evaluated on price, past performance, and whether the offer is technically acceptable considering the minimum requirements included above. Bidders shall quote on all items or none. Please ensure all quotes are complete and accurate, all information is filled out, and required clauses are complied with, otherwise the quotation will be rejected for being non-responsive. The winning contractor will be issued an award corresponding to this solicitation, candidates must include a phone number and/or email address, be registered on the central contractor registration (CCR), ORCA or provide representations and certifications as found in FAR 52.212-3 and 252.212-7000, and not found ineligible for award on the Excluded Partied List System EPLS. The following Air Force Federal Acquisition Regulation (AFFARS) clause applies and is incorporated in full text:     AFFARS 5352.201-9101 OMBUDSMAN (AUG 2005) (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern.
