Description
THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL SERVICES IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 12.6 AND AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. THE SOLICITATION NUMBER IS HSFEMS-08-Q-0015 AND IS ISSUED AS A REQUEST FOR QUOTATION (RFQ). THE SOLICITATION DOCUMENT AND INCORPORATED PROVISIONS AND CLAUSES ARE THOSE IN EFFECT THROUGH FEDERAL ACQUISITION CIRCULAR FAC 05-19. THE NORTH AMERICAN INDUSTRY CLASSIFICATION SYSTEM (NAICS) NUMBER 523490. THE SBA SIZE STANDARD IN MILLIONS OF DOLLARS IS $11.5. THIS IS A 100% SET-ASIDE FOR SMALL BUSINESS FIRMS LOCATED IN THE PRESIDENTAIL DECLARED DISASTER AREA FOR HURRICANE KATRINA. THE FEMA BILOXI TRO HAS A REQUIREMENT FOR ONE FOUR WHEEL DRIVE TELEHANDLER. THE RENTAL OF THE FOUR WHEEL DRIVE TELEHANDLER MAY BE DISCONTINUED AS THE GOVERNMENT'S NEEDS CHANGE. FOR THAT REASON, IT IS REQUESTED THAT THE CONTRATORS PROVIDE A MONTHLY PRICE PER UNIT. THIS CONTRACT WILL BE FOR A BASE PERIOD OF SIX MONTHS WITH A SIX MONTH OPTION PERIOD.
CLIN 0001: PROVIDE RENTAL OF ONE (1) FOUR WHEEL DRIVE TELEHANDLER WITH SATBILIZERS AND A BOOM WITH AN EXTENSION REACH LOAD CAPACITY OF 2,200 LBS., A FORWARD REACH OF 11 FT., AND A MAXIMUM HEIGHT OF 18 FT. PERIOD OF PERFOMANCE WILL BE FOR A BASE PERIOD OF SIX (6) MONTHS. DELIVERY OF TELEHANDLER IS REQUIRED WITHIN THREE (3) DAYS OF CONTRACT AWARD.

The following FAR clauses identified in FAR Clause 52-212-5 are applicable to this acquisition: 52.219-6, Notice of Total Small Business Set-Aside; 52.222-3, Convict Labor; 52.222-19, Child Labor; 52.222-21, Prohibition of Segregated Facilities; 52.222-26, Equal Opportunity; 52.222-35, Equal Opportunity for Special Disabled Veterans, Veterans of the Vietnam Era, and Other Eligible Veterans; 52-225-1, Buy American Act---Supplies; 52-225-13, Restrictions on Certain Foreign Purchases; 52-226-4, Notice of Disaster or Emergency Area Set-Aside; and 52-226-5, Restrictions on Subcontracting Outside Disaster or Emergency Area. A completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items shall be included with the offeror?s quote. Parties responding to this solicitation may submit their quote in accordance with their standard commercial practices (e.g. on company letterhead; formal quote form, etc; but must include the following information: (1) Pricing; (2) Company Tax Information and DUNS Number. CCR Requirement?Contractors must be registered on Central Contractor Registration (CCR) before an award can be made to them. If a contractor is not registered in CCR, they may do so by going to the CCR web site at http://www.ccr.gov.
