The Federal Bureau of Prisons (BOP) is seeking contractual services under the attached solicitation (number RFQ-TDAT-009-08) from sources possessing a current certification to provide outpatient substance abuse treatment and mental health services to Federal offenders confined in community-based programs. These services are required in the geographical vicinity of 801 College St, SE, Grand Rapids Michigan, 49507 located within four city blocks or reasonable walking distance and accessible to a local public transportation route for male Federal offenders. The services are to be provided in accordance with the enclosed Statement of Work entitled, Federal Bureau of Prisons Community Transitional Drug Abuse Treatment and Mental Health Treatment Services dated May 2007 and reviewed June 15, 2007".
The performance period for the resulting contract will be a one-year base period (to begin approximately July 1, 2008) and four additional one-year option periods.
The following MUST be completed and submitted with your quote:
Standard Form (SF) 1449- Solicitation/Contract/Order for Commercial Items - Please complete blocks 17a, 23, 24, 30a, 30b, and 30c of SF-1449 and the continuation sheets. Please include your telephone number in block 17a. Original signatures are required on quotes submitted.
Representations and Certifications (FAR 52.212-3) - Prospective quoters are required to submit Annual Representations and Certifications via the Online Representations and Certifications Application (ORCA) website at http://orca.bpn.gov.
All quoters MUST comply with FAR 52.204-7, Central Contractor Registration and FAR 52.2044-6 Data Universal Numbering System (DUNS) Number.
Quoters are advised to read the entire solicitation to ensure a complete understanding of the terms and conditions and to insure a complete quotation is submitted. Required Documentation identified at FAR 52.212-1, Instructions to Offerors - Submission of Quotes must be followed completely.

Hard copies of the solicitation document will not be available. The site provides downloading instructions.
