To Provide Outreach
Services and Strategies
To Identify
Uninsured Children
The
Department of Human Services, Med-QUEST Division, is requesting proposals from
qualified and properly licensed organizations to:
Identify
uninsured children eligible for medical coverage
Promote effective
outreach strategies through the development various forms of communication
media.
Conduct and host
Statewide Community Training Workshops, Forums and an annual conference of
public/private partnerships.
The
organizations responding to this RFP must be able to provide the required
services on a statewide basis. Proposals
limited to a specific island or county will not be accepted. Only one proposal per legal entity will be
accepted.
Sealed
proposals shall be mailed and postmarked by the United States Postal Service on
or before March 20, 2008, and received no later than 10 days from the submittal
deadline. Hand delivered proposals shall
be received no later than 2:00 p.m., Hawaii Standard Time (HST), on March 20,
2008, by the Med-QUEST Division at 1001
Kamokila Boulevard, Suite 317, Kapolei,
Hawaii
96707. Proposals postmarked or hand delivered after
the submittal deadline shall be considered late and rejected. There are no exceptions to this requirement.
The
Med-QUEST Division will conduct an orientation on February 27, 2008 at 2:00 p.m.
(HST) at the Med-QUEST Office, 601
Kamokila Boulevard, Room 506A, Kapolei, Hawaii.
The deadline
for submission of written questions is March 3, 2008, 2:00 p.m. (HST). All written questions will receive a written
response from the State on or about March 7, 2008.
