Opportunity Summary
The State of Oregon acting by and through its Department of Human Services, Children,
Adults and Families Division, hereinafter referred to as DHS, issues this Request For
Proposals (RFP) for securing a single Contractor who will provide the services described in
this RFP to serve Jackson County. The services sought through this RFP are components of
Family Based Services (FBS): Family Sex Abuse Treatment (FSAT) with After Care Services
(ACS) to DHS Child Welfare Programs. The specific purpose for FSAT: is to resolve the
trauma of incest and help the non-offending parent to make better decisions and prevent
future abuse.

It is the intent of DHS to award a single contract not to exceed $46,609.20 for the initial
contract period that would result in complete coverage of services throughout Jackson
County. Contract award will be for an approximate one year from the effective date of the
contract. DHS reserves the right to amend or extend any resultant contract pursuant to OAR
125-246-0560 as applied and a total contract period of 3 years. Contract amendments or
extension will be based on what is stated in Attachment 2, Exhibit A, Part 3, Section III
Anticipated Amendments.

Within two years, DHS expects programmatic changes made to all services that make up Family
Based Services Program. It is anticipated that through review of best practices within
Oregon and other states, that the Family Based Services model will be modified

DHS reserves the right to amend the statement of work of the resultant contract from this
solicitation to include the additional changes made to the Family Based Services Program.

All persons or firms submitting proposals are referred to as Proposers in this RFP. After
execution of a contract, the awarded Proposer will be designated as Contractor.



Child Welfare Programs are a part of the DHS and support the mission of assisting people to
become independent, healthy and safe. Within DHS, the Children Adults and Families (CAF)
Division is responsible for child welfare programs, including child abuse, investigation &
intervention, foster care, and adoptions. CAF advocates building genuine partnerships with
families that recognize their strengths, in the context of the family's culture. Child
Welfare Program goals include helping to protect a child and promote child safety in the
home by serving families out of local offices throughout the state.

FBS build on family strengths to help meet a child's safety and attachment needs. Services
are provided to prevent placement of children in foster care whenever possible, to help
reunify children placed in foster care, or to help identify alternative permanent homes
when necessary.
