The State
of Hawaii,
Department of Labor and Industrial Relations Office of Community Services
(DLIR-OCS) is soliciting proposals to contract services to provide DLIR-OCS
with an automated statewide client tracking and case management data system for
its employment core milestone payment training programs in accordance with the
Hawaii Public Procurement Code, Section 103D-303, Hawaii Revised Statutes.
DLIR-OCS desires
to contract with an individual or organization with the capability to develop,
install, operate, maintain and provide training on the necessary customized
software.
This customized software will
provide a single cost effective client tracking case management system
( System ). The System will provide the following services: Document services
provided to and track the progress of program clients; incorporate a
centralized intake form; maintain demographic information for program reports;
determine client income eligibility; define client goals; incorporate a
telephone contact module; and use query builders to retrieve records from any
database table meeting DLIR-OCS service providers specified conditions. It should monitor based on scaled client
responses, client progress and program services received to eliminate work
barriers; track client milestone achievements; compile milestone payment
requests and expenditure reports; record telephone contacts, walk-in/general
contacts, and group services (such as classes or workshops); maintain client
confidentiality; and generate real-time monthly, quarterly and annual Milestone
Payment System Performance-Based and Service Activity Reports with vendor
performance report card.

Any person,
firm, or corporation that submits a proposal must, at the time of contract
award, provide the State of Hawaii
with the following:
1.
A tax clearance certificate from the Hawaii State Department
of Taxation and the Internal Revenue Service to the effect that all delinquent
taxes, interests, and penalties levied or accrued against the said person, firm
or corporation have been paid, and/or any other evidence requested by and
acceptable to the contracting officer to demonstrate that the contractor is not
in default on any obligations due to the State of Hawaii, any of its political
subdivisions, and the Internal Revenue Service.
2.
A compliance certificate from the Hawaii State Department
of Labor and Industrial Relations.
3.
A copy of its certificate of good standing from the
Hawaii State Department of Commerce and Consumer Affairs.
The Executive
Director of DLIR-OCS reserves the right to cancel this RFP, to reject any or
all in whole or in part or to waive any defects in any proposal applications in
the best interest of the public.
Campaign
contributions by State and County Contractors. Contractors are hereby notified of the
applicability of Section 11-205.5, HRS, which states that campaign
contributions are prohibited from specified State or County government
contractors during the term of the contract if the contractors are paid with
funds appropriated by a legislative body.
The
contract entered into pursuant to this notice will be awarded to the lowest
responsible contractor subject to availability of funds without discrimination
on the grounds of race, color, religion, national origin or sex. The contractor certifies to the best of
his/her knowledge that the agency is in compliance with certification regarding
Debarment and Suspension; Drug-Free Workplace Requirements; Lobbying; Program
Fraud Civil Remedies Act; Environmental Tobacco Smoke and Americans with
Disabilities Act.
One (1) original and five (5) bound copies
of the proposal in sealed envelopes are to be submitted to DLIR-OCS, Princess
Ruth Keelikolani Building, 830 Punchbowl Street, Room 420, Honolulu, Hawaii
96813, no later than 4:30 P.M. (Hawaii Standard Time), Tuesday July 15, 2008, or postmarked no later than 12:00 midnight,
July 15, 2008. Any mail-ins postmarked
after 12:00 midnight, July 15, 2008, will not be accepted for review and will
be returned unopened.
