| FLORIDA DEPARTMENT OF TRANSPORTATION DISTRICT TWO ADVERTISEMENT FOR DESIGN BUILD € LOW BID CONTRACT NUMBER: E2L31 FEDERAL PROJECT NUMBER: SRTS-037-A FINANCIAL PROJECT NUMBER: 21273355201 PROJECT NAME The Florida Department of Transportation (Department) has issued this Request for Proposal (RFP) to solicit competitive bids and proposals from Proposers for the construction of the Safe Routes to Schools (SRTS) Sidewalk in Suwannee County. DESCRIPTION OF WORK This project consists of constructing concrete sidewalks which includes signing and pavement marking modification for school zones and pedestrian crossings as follows: 1. A 6 foot sidewalk along the south side of Silas Drive from the Suwannee Sports Complex eastward to the east side of Walker Avenue. A special emphasis crosswalk will be provided across Walker Avenue to connect the proposed sidewalk to the existing sidewalk on the east side of Walker Avenue. Side drain extensions and mitered end sections may be required at the entrance drive to the Suwannee Sports Complex. 2. A 6 foot sidewalk along the north side of Primary School Road from Walker Avenue eastward then northward along the west side of Primary School Road to Pinewood Drive. 3. A 6 foot sidewalk along the south side of Marymac Street from Walker Avenue to Suwannee Avenue then south and east along the west and south side of High School Road to Pine Avenue. A 5 foot sidewalk along the west side of Pine Avenue south to Pinewood Way then west along the north side of Pinewood Way to High School Road. A 6 foot sidewalk along the west side of High School Road from Pinewood Way south to Pinewood Drive. 4. A 5 foot sidewalk along the east side of Walker Avenue from the crossing at the south driveway of Suwannee Middle School north to connect to the existing sidewalk along Walker Avenue The project shall include a pedestrian crossing across Marymac Street at Blackburn Street to provide access to the proposed sidewalk from the housing areas located north of Marymac Street. This segment includes a visually identifiable drainage feature along the south side of Marymac Street that will be crossed by the proposed sidewalk and may need a storm sewer pipe and mitered end sections. All school signing shall be brought up to current standards. The current school signing is comprised of older signs using yellow backgrounds. New signing for the improvement shall be comprised of signs with fluorescent yellow-green backgrounds. Existing yellow signs will be upgraded to fluorescent yellow-green. Several existing signs along Pinewood Drive shall be removed to prevent excessive signing of the crosswalks located along the corridor. The two existing crossings located on Walker Avenue at Suwannee Middle School shall be eliminated and combined into one crossing. These crossings currently located within the driveways to the Middle School shall be removed and located further to the south. Relocating the crossing to the south would place it adjacent to an existing stop sign as well as moving it south of the prevailing traffic pattern, which would reduce anticipated conflicts between vehicles and pedestrians. The firm/company shall comply with all jurisdictional, federal, state, county and local regulations and meet appropriate professional design criteria and codes for the state architecturally and mechanically appropriate to ensure a quality design for the project. DESIGN BUILD TEAM REQUIREMENTS: The Contractor and Design professionals cannot be on more than one team. The secondary member (i.e., designer or contractor) of the design build team cannot change, after award, without the written approval of the District Secretary. A professional firm shall not submit a proposal for CEI services, either as a Prime or a Sub, for a Design-Build contract for which the same firm or its affiliate is the Engineer of Record (EOR) or is Sub to the EOR. A consultant firm, its affiliate, or sub consultant that is under contract with the Department to develop the Request for Proposal for a Design Build contract cannot be part of a Design Build Team proposing on that contract as a prime or a sub consultant. A consultant firm, its affiliate, or sub consultant, that is under contract with the Department to provide Construction Engineering and Inspection (CEI) services on the Design Build contract cannot be part of a Design Build Team proposing on that contract as a prime or sub consultant. RESPONSE PROCEDURE: Prequalified firms interested in being considered for the project should request a project package (Project Criteria and Request for Proposal) by submitting the fax request form found on the District Two District Contracts website located at: http://www.dot.state.fl.us/contractsadministrationdistrict2. RESPONSE DUE DATE: Project packages will be mailed to those firms that have requested them on or before April 25, 2008. This Project has a Mandatory Pre-Proposal Conference on April 25, 2008 at 10:00 a.m. at the Lake City District Office, 1109 S. Marion Avenue, Lake City, Florida. Plans, Specifications and Proposal Packages will no longer be available after the Mandatory Pre-Proposal Conference. NOTE: Bid proposals with technical proposals will be due to the District Contracts Office on May 20, 2008 on or before 2:00 p.m. No late bids with technical proposals will be accepted after the 2:00 p.m. deadline of the bid due date. SELECTION PROCEDURE: Firms shall submit a bid proposal and a technical proposal. All bids will be opened and the technical proposal of the lowest bidder shall be evaluated by the FDOT to determine if it is responsive and responsible. If the proposal is deemed non-responsive or non responsible, the technical proposal of the next lowest bidder will be evaluated. The Department shall reserve the right to reject any and all proposals and waive minor proposal irregularities.
