STATEMENT OF OBJECTIVES
STRATEGIC PLANNING FACILITATOR
Facilitator needed for guiding the 45 members of the Fort Defiance Indian Health Services Dental Staff in developing a five-year strategic plan. The plan needs to include a mission statement, goals and plans. Facilitator shall provide references to show evidence of facilitating similar strategic planning sessions within the past three years.
The facilitator will encourage and elucidate participation by all staffing categories in the dental clinic: basic dental assistants, expanded function dental assistants, pediatric dental assistants, supervisory dental assistants, dental hygienists, front desk receptionists, secretary, dentists, specialty dentists, and supervisory dentists.
Sessions will be four hours in length on Thursday morning(s) at a facility provided by FDIH. It is anticipated that no more than four sessions will be necessary. The facilitator will provide a proposal outlining the processes to be used at each sessions and the total number of sessions that are recommended to achieve the requested strategic plan. The sessions will be arranged on dates that are mutually agreeable to FDIH and the facilitator within the shortest time span possible, not to exceed 12 weeks.
The facilitator will provide written and electronic documentation that records the process and the resultant final strategic plan (including mission statement, goals and plans). This will be provided within one week of completing each session, and within two weeks of completing the overall process.
The facilitator will provide all supplies needed for the process (such as, but not limited to pens, markers, paper, easels, large display papers, tape, "post-it" notes, handouts, warm-up exercises, participation incentives). Any common electrical audio-visual equipment that the facilitator requests FDIH to provide will be listed in the proposal, otherwise all electrical audio-visual equipment will be provided by the facilitator.
The facilitator will provide transportation, meals, telephone and lodging.
