This is a combined synopsis/solicitation for the establishment of multiple Blanket Purchase Agreements for Insignia Sewing and Embroidery Services prepared in accordance with the format in FAR 13.303, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W91248-08-T-0043 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. The NAICS Code is 315299 and the small business size standard is $6.5 million. The period of performance is one twelve month period. Wage Determination 2005-2187, REV 5, dated 05/29/08 applies.. EFFECTIVE PERIOD: This Blanket Purchase Agreement is effective from date of award of BPA through 31 August 2009.

The BPA pricing for any item will be considered a valid quote from your firm for the sewing or embroidery of that item until the expiration date stated on your BPA. A call may be placed at any time during that period for the item at the price agreed on the BPA. Also, in accordance with FAR 16.702(b)(2) each basic agreement shall provide for discontinuing its future applicability upon 30 days written notice by either party..

The quantities stated below are only ESTIMATED and may not reflect actual Calls placed. Individual calls shall not exceed the simplified acquisition threshold, which is $100,000.00.

Contractors are not expected to be capable of performing on every item listed at all times. Contractors have the option of bidding on only one item, several items, or all items.

The BPA will be established first, than a call; will be placed or awarded, based on the lowest price that can be found to be fair and reasonable.

The goal is to establish several BPAs with multiple contractors


STATEMENT OF WORK

INSIGNIA SEWING
SCOPE

INSIGNIA SEWING
1. The contractor shall provide insignia sewing service for Fort Campbell, KY uniforms
The authorized government caller (Contract Officer Representative COR) will coordinate
with unit point of contact (POC) to deliver clothing and insignia to be sewn to the
contractor. The unit may require patches sewn on Cook White uniform shirts. All
insignia for the Army Combat Uniform (ACU) is to be affix to the hook side Velcro.
Sewing of rank on soft cap, kelvar cover, and booney hats when requested. The COR
will specify type of shirt/hat, quantity, and estimated insignia prior to turn-in.

2. The contractor shall affix insignia to the uniform garment as specified and depicted in
the placement illustration contained in AR 670-1 (Wear and Appearance of the Army
Uniform and Insignia).

3. The turn around time will vary due to unit mission requirements. The estimated range
may be as short as two (2) working days maximum of several weeks for completion of
up to 15,000 20,000 patches

4. Accountability. The contractor shall provide a list with the individual name, how
many patches, how many shirts and total cost. List will be submitted with the invoice.

EMBROIDERED NAME TAPES/US ARMY
1. The contractor shall provide embroidered name/us army tapes for Fort Campbell, KY
Uniforms. The COR will coordinate with the unit POC to deliver names and quantity to
be made to the contractor. The COR will notify the contractor of the embroidered tape
color. Name/US Army tapes for the ACU uniform will be embroidered on the digital
ACU material and then affixed to the hook side Velcro.

2. The turn around time will vary due to unit mission requirements.

3. Accountability. The contractor shall provide a list with the individual name, quantity,
unit cost, and total cost. List will be submitted with the invoice.

4. Invoice. The invoice will contain the contractor name, address, phone number,
contract number, call number, CLIN number and title with total quantity, unit price,
total price, and a signature block. (AFTER VERIFICATION OF ACCURACY,
INVOICE WILL BE PAID BY THE COR WITH THE GOVERNMENT CREDIT
CARD).

EXTENT OF OBLIGATION: The Government is obligated only to the extent of
Authorized calls actually placed against the agreement. This BPA does not obligate
Funds.

PURCHASE LIMITATION: No individual call shall exceed $25,000.00.

MILAGE LIMITATION: Contractor shall be within 10 miles of Gate 4, Fort Campbell,
KY.
