Description
Individual Personal Service Notice - Registered Pharmacist.
PERIOD OF PERFORMANCE: 12 MONTHS
SCOPE OF CONTRACT:    Contractor shall provide pharmacy services for and on behalf of the U.S. Government in accordance with (IAW) the laws and regulations in effect on the execution of this contract, and as they may be amended from time to time.    Services rendered shall be of a professional quality at a minimum meeting current recognized national standards as established by the Joint Commission on Accreditation of Health Care Organizations (JCAHO), the Accreditation Association for Ambulatory Health Care (AAAHC), and the Standards of Practice, American Society of Health-System Pharmacists.    The Contractor shall perform services IAW the ethical professional and technical standards of the health care industry, and in compliance with all applicable law and regulation.
1.    CONTRACTOR QUALIFICATIONS:    Individual performing services under this contract shall meet the following minimum qualifications:
1.1.1. A Bachelors, Masters, or Doctoral degree in Pharmacy from a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE).
1.1.2. A current, valid, unrestricted license to practice Pharmacy in any one of the fifty States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands.
1.1.3.    Experience as a registered pharmacist for at least 24 consecutive months within the last 36 months. The Pharmacy Chief in consideration of other experience, education, training, or combination thereof may waive this requirement
1.2. REQUIRED DOCUMENTATION OF QUALIFICATIONS FOR PHARMACISTS:
1.2.1.      Pharmacists shall provide a copy of a diploma issued to the individual from an accredited college of pharmacy.
1.2.2.    Pharmacists shall provide a copy of their current license renewal certificate.
1.2.3. Pharmacists shall provide a copy of their position history covering the most recent five years.
1.3. DISQUALIFIERS:
1.3.1. No pharmacist who is chemically dependent or impaired shall be acceptable.
1.3.2. No pharmacist who has been previously convicted of a felony shall be acceptable.
1.3.3. No pharmacist who is awaiting possible adverse administration action by the Board of Pharmacy or a current employer shall be acceptable.
